Anesthetic Management of the Child with Osteogenesis imperfecta: Seven General Anesthesia Experience in One Patient

Osteogenezis İmperfektalı Çocuk Hastada Anestezi Yönetimi: Tek Hastada Yedi Genel Anestezi Deneyimi

Abstract

Osteogenesis imperfecta is a rare , inherited genetic disor-der of connective tissue which is characterized by abnormal Type I collagen production. This causes decrease in bone mass of human body, bones become fragile and brittle, re-sulting in unreasonable multiple fractures. Other manifes-tations include hyperextensibility of the joints, blue sclera, hearing loss, short stature and dentinogenesis imperfecta. During the anesthetic management of these patients, intu-bation difficulties, cervical spine fractures, and malignant hyperthermia triggered by used anesthetic agents may de-velop. Anatomic deformities and platelet dysfunction influ-ence the choice of anesthetic technique. We aimed to pre-sent a series of 7 general anaesthetics in one child patient suffering from this disease along with the literature.

Özet

Osteogenesis imperfekta, anormal Tip I kollajen üretimi ile karakterize kalıtımsal, ender görülen bağ dokusu hastalığıdır. Kemik kütlesinde azalma, kırılgan ve gevrek kemik oluşumu sonucunda artmış multiple fraktürler görülür. Eklemlerin hiperekstansibilitesi, mavi sklera, işitme kaybı, kısa boy ve dentinogenesis imperfekta Osteogenesis imperfektada görülen diğer klinik bulgulardır. Bu hastalarda anestezi uygulamaları sırasında, entübasyon güçlüğü, servikal vertebra fraktürleri ve kullanılan anestezik ajanlara bağlı malign hipertermi gelişebilir. Anatomik deformiteler ve trombosit disfonksiyonu anestezi tekniği seçimini etkilemektedir. Biz Osteogenesis imperfektalı bir çocuk hastanın geçirdiği 7 genel anestezi deneyimini literatür eşliğinde sunmayı amaçladık
