Simultaneously Emerging Multiple Trigger Fingers in Both Hands

Abstract

Objective: Trigger finger, which is one of the most common reasons of pain and malfunction of the hands, emerges with pain, triggering or limited extension during the movements of flexor tendons and may effect all fingers. Aim of this study is to present a rare case of simultaneously emerging multiple trigger fingers in both hands. Case Report: A 58 year old female patient has attended to our clinic with pain and triggering in her third, fourth and fifth fingers of left hand and fourth and fifth fingers of her right hand. She has been diagnosed as multiple trigger fingers. Patient is operated under local anesthesia from her left hand. In the postoperative third month control of patient, we observed that all complaints of the patient were resolved. Operation was planned for the right hand. Discussion: It is known that success ratio of steroid injection is low in patients who have multiple trigger fingers, long lasting symptoms and history of diabetes mellitus. Surgical therapy is the treatment of choice in patients with trigger finger because of the short operation time under local anesthesia, low recurrence and complication rates of the surgery. Conclusion: Surgical therapy is superior to local steroid injections in patients with multiple trigger fingers who have concomitant systemic diseases and ongoing symptoms more than six months because of high success and low recurrence rates in single session.

Özet

Amaç: Elde ağrı ve fonksiyon kaybının en sık nedenlerinden olan tetik parmak, fleksör tendonun hareketi sırasında ortaya çıkan ağrı, takılma veya parmakta ekstansiyon kısıtlılığı ile bulgu vermekte olup, tüm parmakları etkileyebilmektedir. Bu çalışmada, nadiren ortaya çıkabilen her iki elde eşzamanlı çoklu tetik parmak olgusunun sunulması amaçlanmıştır Olgu Sunumu: Her iki el parmaklarında hareketle ağrı ve takılma yakınmaları ile başvuran 58 yaşında kadın hastanın yapılan muayenesinde, sol elin üçüncü, dördüncü ve beşinci parmakları ile sağ elin dördüncü ve beşinci parmağında bulgular tetik parmak ile uyumluydu. Lokal sedasyon altında sol elinden opere edilen hastanın postoperatif 3. ayda yapılan kontrolünde yakınmaların tamamen ortadan kalktığı görüldü. Hastanın sağ eline yönelik operasyonu planlandı. Tartışma: Birden fazla parmakta tetiklenme yakınması olan, semptomları uzun süredir devam eden ve diyabetes mellitus öyküsü bulunan hastalarda steroid enjeksiyonu başarı yüzdesinin düşük olduğu bilinmektedir. Lokal anestezi ile yapılacak kısa bir operasyon olması, düşük nüks ve komplikasyon oranları ile yüksek başarı oranları nedeniyle cerrahi tedavi, tetik parmak tedavisinde ideal tedavi seçeneğidir. Sonuç: Eşlik eden sistemik hastalığı bulunan, semptomları 6 aydan uzun süredir mevcut çoklu tetik parmak olgularında cerrahi tedavi, steroid tedavisine göre tek seansta sağ- ladığı yüksek başarı oranı ve düşük nüks riski nedeniyle ön planda yeğlenmelidir.
