Results of oral acyclovir treatment in Herpes zoster oticus

Abstract
Incidence of facial paralysis due to Herpes zoster oticus (Ramsay Hunt's syndrome) is not common and its response to steroid therapy is not as good as Bell's palsy. In this study the results of acyclovir treatment in 7 patients with Herpes zoster oticus were presented. Diagnosis was based upon severe pain and vesicles around the involved ear and the appearance of facial paralysis which developed on the same side on first or second days of disease. Oral acyclovir was administered as 4x800 mgr/perday for ten days. Vesicles around the ear were disappeared on fourth or fifth days after the beginning of the treatment. In 5 patients facial paralysis with a pretreatment level of House-Brackmann evre-lll and IV was completely improved after the treatment. One of the two patients who have evre-V facial paralysis demonstrated a significant improvement (evre-ll) and the other one underwent to total facial decompression due to proved no healing. In conclusion, acyclovir was seen to be effective for relief of pain and for resolution of the vesicles around the ear and improvement of the facial paralysis which results from Herpes zoster oticus.

Herpes zoster otikus'a (Ramsay Hunt sendromu) bağlı fasial paralizinin görülme sıklığı az ve steroid tedavisine verdiği cevap Bell paralizi kadar iyi değildir. Bu çalışmada son 6 yıl içinde kliniğimizde Herpes zoster otikus tanısı konan 7 hastada acyclovir tedavisi ile elde edilen sonuçlar sunulmuştur. Tanı, tutulan kulak çevresinde şiddetli ağrı, kulak sayvanında veziküller ve aynı tarafta 1-2 gün içinde gelişen periferik fasial paralizinin varlığı ile konmuştur. Tedavi için, günlük dozu toplam 3.2 gram olacak şekilde oral (4x800 mg/gün) acyclovir 10 gün süre ile uygulanmıştır. Tedavinin başlangıcından sonraki 4-5. günler içinde kulak çevresindeki veziküller iz bırakmadan kaybolmuştur. Fasial paralizisi House-Brackmann evre-lll ve IV olan 5 hastada tam iyileşme izlenirken, evre-V düzeyinde fasial paralizisi olan iki hastadan birinde de önemli ölçüde düzelme görülmüş (evre-ll), 1 olguda düzelme olmadığından total fasial dekompresyon yapılmıştır. Sonuç olarak acyclovir'ın Herpes zoster otikuslu hastalarda hem kulak çevresindeki ağrı ve veziküllerin iyileşmesinde, hem de fasial paralizinin düzelmesinde etkili olduğu görülmüştür.
