Early determinated sensory-neural hearing loss by electrophysiological testing in patients with diabetes mellitus

Abstract

22 patients with Diabetes Mellitus who have no complaints of hearing loss underwent Brainstem Auditory Evoked Response (BERA) and Distortion Product Otoacoustic Emission (COPOAE) tests. Findings of these patients were compared with the findings of the control-group that consisted of 10 healthy subjects. BERA results of diabetic patients showed elongation of the peripheral and central nerve transmission time compared with the control group. With DPOAE, cochlear pathology is also demonstrated. The elongation of the peripheral nerve transmission time of diabetic patients is thought to be in relation with microangiopathy and central nerve transmission time with the neuropathy. The correlations between BERA and DPOAE values and duration, therapeutic control and modality of the treatment of Diabetes Mellitus are also investigated. The effectiveness of BERA and DPOAE in early stage (subclinic) hearing loss is emphasized.

Özet

İşitme kaybı yakınması olmayan, saf ses ve impedans odyometrisi normal bulunan 22 diabetes mellituslu hasta, beyin sapı işitsel uyarılmış potansiyelleri (BİUP) ve distortion product otoakustik emisyon (DPOAE) testleri ile değerlendirildi. Bulgular, 10 sağlıklı kişiden oluşan kontrol grubuna ait sonuçlarla karşılaştırıldı. Diabetik hastalara ait BİUP ölçümlerinde elde ettiğimiz değerler, periferik iletim zamanı ve santral iletim zamanında uzama şeklinde görüldü. DPOAE ölçümleri ile de kohlear patoloji daha açık bir şekilde tespit edildi. Periferik iletim zamanındaki uzama diabetes mellitusdaki mikroanjiopati ile, santral iletim zamanındaki uzama da nöropati ile ilişkilendirildi. BİUP ve DPOAE değerleri ile diabet süresi, diabetin kontrolü ve tedavi şekli arasındaki ilişki araştırıldı. BİUP ve DPOAE ölçümlerinin erken dönem (subklinik) işitme kayıplarındaki değeri vurgulandı.
