The role of L-arginine, a nitric oxide precursor, on deteriorating cardio-respiratory effects in experimentally produced traumatic diaphragmatic injury

Abstract
Aim: In this study we aimed to investigate the protective effects of L-Arginine on cardio-respiratory complications in a diaphragmatic rupture model. Method: 40 Sprague-Dawley rats were used in this study. In Group I, only thoracotomy was performed. In Group II, HI and IV, following left thoracotomy, the stomach was pulled into thorax and the intraabdominal pressure was increased by an insufflator. L-Arginine (L-Arg) was given in GUI and L-NAME was given in GIV at the 45th minute of the study. The parameters such as arterial blood pressure (BP), heart rate (HR), arterial blood gases analysis (pC>2, pCC>2, pH) and pulmonary scintigraphy were measured at the baseline, 30tn, 45!" and 60^ minutes of the study. The left lungs were extracted for histopathological examination. Results: In the values of blood gases analysis following the performing herniation model the pO2 and pH values decreased and pCC>2 values increased in Gil in comparison with GI. The mean values of blood gases analysis in GUI following L-Arg infusion at the 60tn minute of the study were found to be increased for pO2 and pH values and to be decreased for pCO2 values in comparison with the values of GII. Following L-NAME administration to the GIV, there was no difference found between GII and GIV. Pulmonary uptake decreased in GII, GIII and GIV when compared with GI, whereas pulmonary uptake in GIII was significantly increased in comparison with GII and GIV. Conclusion: The use of the precursor of nitric oxide, L-Arginine, may provide protective effects on cardio-respiratory complications related to organ herniation in the acute period of traumatic diaphragmatic rupture.

Amaç: Bu çalışmanın amacı deneysel diafragma rüptüründe kardiyo-respiratuar fonksiyonlar üzerine L-Argininin koruyucu etkilerini incelemektir. Yöntem: Çalışmada 40 adet Sprague-Dawley cinsi sıçan kullanıldı. Grup l; Sadece torakotomi yapıldı. Grup 11, IH ve IV'de sol torakotomi sonrası mide toraks içine çekildi ve insııflatör kullanılarak karın içi basıncı yükseltildi. Glll'de 45. dk'dan itibaren L-Arginine (L-Arg), G IV'de ise L-NAME verildi. Tüm gruplarda bazal, 30, 45 ve 60. dk'larda arteriyal kan basıncı, kalp atımı, arteriyal kan gazı (PaO2, PaCC>2, pH), akciğer sintigrafi çalışması ve histopatolojik inceleme yapıldı. Bulgular: GII sıçanlarda mide herniasyonu ve karın içi basıncının artırılmasından sonra GI ile karşılaştırılmasında PaO2, ve pH değerlerinde düşme ve PaCO^de yükselme saptandı. GIII sıçanlarda L-Arg infüzyonu sonrası 60. dk'da ölçülen kan gazı değerlerinin GU'deki değerlerle karışlaştırılmasında ise PaC>2, ve pH değerinde yükselme ve PaCO^de düşme saptandı. GIV sıçanlarda L-NAME infüzyonu sonrası ölçülen kan gazı değerlerinin GII ile karşılaştırılmasında değişiklik saptanmadı. Akciğer sintigrafisinde GII, GIII ve GIV de kontrol grubuna göre pulmoner aktivite tutulumunun azaldığı, GIII ile GII ve GlV'ün karşılaştırılmasında pulmoner aktivite tutulumunun belirgin arttığı saptandı. Sonuç: Çalışmamızda, sıçanlarda diafragma rüptürü sonrası akut dönemde organ herniasyonuna bağlı ortaya çıkan kardiyo-respiratuar bozukluklar üzerine NO prekürsörü L-Arginin iyileştirici yönde etkisinin olduğu gözlenmiştir.
