Is routine preoopreative testing indicated for children with inguinal hernia?

Abstract
Aim: There are different practices of preoperative routine testing for outpatients. In this study, the incidence of coexisting diseases that increases relative risk of anesthesia and the usefulness of the routine preoperative testing before outpatient inguinal hernia operations have been investigated. Method: We retrospectively reviewed the records of 134 patients operated with diagnosis of inguinal hernia between January 2000 and May 2002. Routine analyses such as complete blood count, blood chemistry, chest x-ray and coagulation tests were performed in all patients after physical examination. Patients who had normal test results (group A) and abnormal test results were compared for the time interval between admission and operation day, and age groups. Results: There were 93 boys and 41 girls with a mean age of 3.6 years (12 days-15 years). Of the 134 patients, 24 (17.9 %) had aberrant laboratory values or radiological findings that increase risk of general anesthesia. Lower and upper respiratory tract infections, and anemia were the most commonly detected coexisting diseases. The mean time between admission and operation day was 8.1 days in group A and 21.6 days in group B (p<0.05). The mean age was 4 years in group A, whereas it was 1.75 years in group B (p<0.05). The percentage of patients who are younger than 2 years was significantly higher in group B. Conclusion: The presence of coexisting disease lengthens the time period between the day of diagnosis and the day of operation. In the regions where the primary health care facilities are limited, the coexisting diseases that increase the risk of general anesthesia should be investigated. A careful medical history and physical examination may decrease the number of preoperative tests.

Amaç: Günübirlik ameliyatlar öncesi rutin inceleme istenmesi konusunda farklı uygulamalar vardır. Bu çalışmada, kasık fıtığı ameliyatı planlanan hastalarda genel anestezi riskini artıran yandaş hastalıkların oranı ve bunların saptanmasında rutin incelemelerin rolü araştırıldı. Yöntem: Ocak 2000-Mayıs 2002 tarihleri arasında kasık fıtığı nedeniyle ameliyat edilen 134 hastaya ait bilgiler geriye dönük olarak incelendi. Hastalara muayene sonrası tam kan sayımı, kan biyokimyası, akciğer grafisi ve koagülasyon testleri gibi rutin incelemeler yapıldı. Laboratuvar ve radyolojik inceleme sonuçları normal olan hastalar (Grup A) ile anormal olan hastalar (Grup B), başvuru ile ameliyat arasında geçen süre ve yaş grupları yönünden karşılaştırıldı. Bulgular: Hastaların 93'ü erkek, 41'i kız, ortalama yaşları 3.6 yıl (12 gün-15 yıl) idi. Toplam 134 hastanın 24'ünde (% 17.9) anesteziye engel olacak anormal test sonucu ve radyolojik bulgu saptandı. Bunlar sıklık sırasına göre alt ve üst solunum yolları infeksiyonları ve anemiydi. Grup A'da başvuru ile ameliyat arasında geçen süre ortalama 8.1 gün iken grup B'de bu süre 21.6 gün olarak belirlendi (p<0.05). Yaş ortalamaları grup A'da 4, grup B'de 1.75 yıl olarak bulundu (p<0.05). Grup B'deki 24 hastanın 17'si (% 70.8) 2 yaş altında iken bu oran grup A'da % 38.2 olarak bulundu (p<0.05). Sonuç: Olgularda yandaş hastalıkların varlığı, tanı ile ameliyat arasında geçen süreyi uzatmaktadır. Temel sağlık hizmetlerinin yaygın olmadığı bölgelerde günübirlik ameliyat planlanan hastalarda, anestezi riskini artıran yandaş hastalıklar araştırılmalıdır. Ayrıntılı öykü ve muayene ile ameliyat öncesi inceleme sayısı azaltılabilir.
