Assessment of endometrium by vaginal ultrasonography in postmenopausal vaginal bleeding

Postmenopozal vajinal kanamalarda endometrium kalınlığının vajinal ultrasonografi ile değerlendirilmesi

Abstract

A total number of 63 postmenopausal women with vaginal bleeding were admitted to our polyclinic. Vaginal ultrasonography was performed as the maximal endometrial thickness at longitudinal axis, to all patients before fractional curettage. It was found that the mean age was 51±7.16 (39-77) and the mean of the postmenopausal period was 11.8 ± 2.83 (9-25) years. Of 41 women with an endometrium < 5 mm, 36 women (%87.80) had atrophic endometrium, 2 women (%4.87) had proliferative and secretory endometrium, 3 women (%7.31) had insufficient matherial in endometrial hystologt. It was not found endometrial hyperplasia, polyps and cancer. Of 22 women with an endometrium > 5 mm, 1 (%4.54) woman atrophic endometrium, 9 (%40.9) had proliferative and secretory endometrium, 1 (%4.54) woman had insufficient material, 7 (%31.81) women had endometrial hyperplasia, 2 (%9.09) had endometrium adenocancer. When endometrial thickness was $\leq$ 5 mm, hystologically 87.8 % atrophic endometrium was found. There was no endometrium adenocancer.

Postmenopozal dönemde vajinal kanama şikayetiyle polikliniğimize müracaat eden, toplam 63 olgu çalışmaya alındı. Bu olgulara fraksiyone küretajdan önce, vajinal ultrasonografi ile longitudinal aksda en kalın endometrium kalınlığı ölçüldü. Olguların yaş ortalaması 51±7.16 (39-77) ve ortalama postmenopozal süreleri 11.8 ± 2.83 (9-25) yıl olarak bulundu. Histopatolojisi incelenen ve endometrium kalınlığı $\leq$ 5 mm olan 41 olgunun 36'sı atrofik endometrium (% 87,80), 2'si proliferatif ve sekretuar endometrium (% 4,87), 3'ü de yetersiz materyal (%7,31) olarak gelmiştir. Endometriyal hiperplazi, polip ve adenokanser saptanmamıştır. Endometrium kalınlığı > 5 mm olan 22 olgunun ise 1'inde (% 4,54) atrofik endometrium, 9'unda proliferatif ve sekretuar endometrium (% 40,9), 1'nde yetersiz materyal (%4,54), 7'sinde endometriyal hiperplazi (%31,81), 2'sinde endometriyal polip (% 9,09), 2'sinde de endometrium adenokanseri (% 9,09) saptanmıştır Endometrium kalınlığı 5mm ve altında olan olgularda % 87,8 oranında atrofik endometrium bulduk. Hiçbir endometrium adenokanseri saptamadık.
