Effects of hemodialysis and hemodialysis membranes on serum selenium and malondialdehyde levels in patints with choronic renal insufficiency

Abstract

Lipid peroxidation is a catalytic event in which the free oxygen radicals produced externally or internally causes the damages on the biomembranes and their major components phospholipids. One of the end products of this process is MDA. This pathological pathway can be prevented by some physiological defence systems that one of them consists of glutathione peroxidase enzymes and its cofâctor Se. There are evidences that lipid peroxidation causes the complications observed in patients with chronic renal insufficiency. In this present study, we measured MDA levels by using thiobarbituric acid method Se levels with atomic absorption spectrophotometry in patients with chronic renal insufficiency before and after hemodialysis. We also compared the results of patients with 20 healthy controls. At the same time, we evaluated the effects of different dialyse membranes on those parameters. The mean MDA level in patients before dialysis were significantly higher than that of the control group(p<0,001). For dialysis group in which polysulphone and celulose acetate membranes were used, MDA level was significantly higher than the predialysis group(p<0,001). There were not significant difference between MDA levels obtained after polysulphone and celulose acetate niembranes(p>0,05). Although dialysis caused an increase in MDA levels, we showed that Se was not lost through dialysis membranes. As a result, while dialysis caused an increase MDA levels, Se was not lost through the dialysis membranes.

Özet

Lipid peroksidasyonu; eksternal ve internal kaynaklardan oluşturulan serbest oksijen radikallerinin, biomembranlarda ve bunların majör komponenti olan fosfolipidlerde harabiyet meydana getirdiği katalitik bir olaydır. Bu kimyasal sürecin son ürünlerinden biri de Malondialdehit (MDA)dir. Bu patolojik döngü bazı fizyolojik savunma mekanizmaları tarafindan ödenebilmektedir. Bu mekanizmalardan bir tanesi Glutatyon Peroksidaz enzimi ve kofâktörü olan Selenyum(Se)dur. Kronik renal yetmezlikli hastalarda görülen komplikasyonlara, artmış lipid peroksidasyonun sebep olabileceğine dair literatür bilgileri vardır. Biz de bu çahşmada,kronik renal yetmezlikli, dializ korumasındaki 30 hastada tiyobarbitürik asit metodu ile MDA ve atomik absorbsiyon spektrofoto-metresi ile Se seviyelerini dializ öncesinde ve sonrasında tayin edip, 20 sağlıklı kontrolle karşılaştırdık ve farklı dializ membranlarının bu parametreler üzerindeki etkilerini izledik. Dializ öncesi MDA düzeyi kontrol grubundan anlamlı derecede yüksekti (pO.OOl). Hem polisulfon, hem de selüloz asetat membranlarının kullanıldığı dializlerde, MDA düzeyi dializ öncesine göre daha yüksekti (p 0.05). Dializ uygulaması MDA seviyesini arttırıcı etki yaparken, dializ membranlan yolu ile Se kaybının olmadığı tespit edildi. Sonuç olarak dializ uygulaması; MDA seviyesine artırıcı etki ya-parken,dializ membranlan yolu ile selenyum kaybı olmadığı sonucuna varıldı.
