Renomegaly in cases of acute lymphoblastic leukemia

Abstract

Renal infiltration in ALL is generally asymptomatic, and can be detected as renomegaly by ultrasound. Renal enlargement in the literature is revealed as 18% in newly diagnosed ALL patients. Seventy ALL patients diagnosed and followed at DR. Behçet Uz Children's Hospital Haematology department were evaluated by ultrasound and renal enlargement was detected in 7. Convantionel tomography confirmed the finding of renömegaly in 3 cases. Renomegaly was, found as 10% in the cases evaluated. Two cases were evaluated as SRG, one as MRG and four as HRG. ALL-BFM 90 therapy protocol was performed in 5 cases, UKALL therapy protocol in 3 cases. In of all the cases, renömegaly resolved at the end of the indiction therapy. All cases of renömegaly are in remission. Now, SIX patients are in the maintanence therapy and one patient is off-therapy.

Özet

Akut lenfoblastik lösemide (ALL) renal infiltrasyon genellikle asemptomatik olup, ultrasonografı (USG) de renomegali olarak belirlenebilir. Çalışmalarda yeni tanı ALL'li olgularda renomegali oranı %18 olarak verilmektedir. Dr. Behçet Uz Çocuk Hastanesi Hematoloji bölümünce izlenen 70 ALL 'H olguya tanı anında batın USG ' si yapılmış ve 7 olguda renomegali saptanmıştır. Bu olgulardan 3 'ünde konvansiyonel tomografi ile renomegali desteklenmiştir. Değerlendirmeye alınan olgularda renomegali sıklığı %10 olarak bulunmuştur. Olguların 2'si standart risk (SRG), biri intermediate risk (MRG) ve 4'ü ise yüksek risk grubu (HRG) olarak değerlendirilmiştir. 5 olguya ALL BFM 90, 2 olguya ise UKALL protokolü uygulanmıştır. İndüksiyon bitiminde olguların tümünde renomegali kaybolmuştur. Renomegali saptanan tüm olgular remisyonda olup; 6 olgu idame tedavisi almakta, bir olgu ise tedavisi kesilmiş olarak izlenmektedir.
