Serologic screening for coeliac disease in school children with dental enamel lesions

Abstract

We formerly reported dental enamel defects in 37% of our coeliac patients (15/41). In this study, we investigated the relation between enamel lesions in healthy schoolchildren and coeliac disease. Enamel hypoplasia and defects were seen in 58 of 1361 students (4.2%) at two elementary schools by screening. Sérologie tests for coeliac disease were performed in 38 students have informed consent of the 58. Gliadin antibodies IgA or IgG were found positive in two children. Jejunal biopsies showed normal mucosal architecture in these two cases. Reticulin IgA and endomysium IgA antibodies were all negative in 23 of the 38. We did not found coeliac disease in 38 schoolchildren with enamel pathologies. Noticeable dental enamel lesions in coeliac patients, and the converse, no one coeliac patient in healthy children with the enamel lesions, were observed.

Özet

Daha önce çölyak hastalarımızın %37'sinde (15/41) enamel defektleri olduğunu bildirdik. Bu çalışmada sağlıklı okulçocuklarında görülen enamel lezyonları ile çölyak hastalığı arasındaki ilişkiyi araştırdık. Enamel hipoplazisi ve defektleri, diş sağlığı için taranan iki ikokuldaki 1361 öğrencinin 58'inde (%4.2) görüldü. Bu 58 öğrenciden izin verilen 38' inde çölyak için serolojik testler yapıldı. Gliadin IgA veya Gliadin IgG, iki çocukta pozitif idi. Bu iki olgudan alınan jejunum biyopsilerinde normal mukoza yapısı bulundu. Reticulin IgA ve Endomysium IgA antikorları da araştırılan 23 öğrencinin tümünde normal bulundu. Enamel patolojileri olan 38 okulçocuğunda çölyak hastalık saptamadık. Çölyak hastalarında önemsemeye değer oranda dental enamel lezyonlar görülmesine karşın enamel lezyonları olan sağlıklı çocuklarda hiçbir çölyak olgusu gözlenmedi.
