A seronegative rheumatoid arthritis case with rheumatoid nodules

Romatoid nodüllerle başvuran ve romatoid faktör negatif saptanan bir juvenil romatoid artrit olgusu

Abstract

Rheumatoid nodules are not so common in juvenile rheumatoid arthritis and other rheumatologic diseases of childhood onset. In this study a twelve year old girl admitted to our hospital because of morning stiffness, polyarthralgia and multiple subcutaneous nodules and diagnosed as Juvenile Rheumatoid Artritis (JRA) has been discussed. She had been suffering from arthralgia of upper and lower extremities following Scarlet Fever. Then subcutaneus nodule formations of different sizes occured at the extansory surfaces of metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints, occipital area, extansory surface of right elbow. Laboratory analysis revealed acute phase response, hypergammaglobulinemia, negative rheumatoid factor (RF) and antinuclear antibody (ANA). In follow up she developed polyarthritis and she was evaluated as RF negative polyarticular JRA. The biopsy of the nodule confirmed the diagnosis. Since RF negativity accompanied by the nodules is rather rare, this interesting case has been presented.

Özet

Romatoid nodul; juvenil romatoid artrit ve diğer juvenil başlangıçlı romatolojik hastalıklarda seyrek rastlanan bir bulgudur. Bu çalışmada poliartralji, sabah tutukluğu ve multipl deri altı nodülleri ile hastanemize başvuran ve Juvenil Romatoid Artrit (JRA) tanısı alan 12 yaşındaki bir kız olgu tartışılmıştır. Hastamızda kızıl enfeksiyonu izleyerek alt ve üst ektremite eklemlerinde ağrı ve hareket kısıtlığı geliştiği, ardından bilateral el metakarpofalangeal (MKF) ve proksimal interfalangeal (PIF) eklem ekstansör yüzlerinde, oksipital bölgede, sağ dirsek ekstansör yüzde değişik büyüklüklerde deri altı şişlikleri oluştuğu öğrenilmiştir. Laboratuar incelemelerinde akut faz reaktanlarında yükselme, hipergammaglobulinemi saptanmış, romatoid faktör (RF) ve anti-nükleer antikor (ANA) negatif bulunmuştur, İzlem sırasında poliartrit gelişen hasta RF (-) form ve romatoid nodul birlikteliğinin çok nadir olması nedeniyle olgu ilginç bulunarak sunulmuştur.
