The effects of tranexsamic acid bolus dose and infusion treatment on postoperative bleeding and hematologic parameters following cardiopulmonary bypass ( Randomized controlled trial )

Abstract

Tranexamic acid (TA) has been shown to decrease bleeding in CPB although the ideal administration regimen is still not clear. In this randomised controlled clinical study, TA infusion was compared with bolus administration in terms of postoperative bleeding and transfusion requirements.Material and Methods: 45 Adult patients undergoing elective CABG vere enrolled in the study. Group I (n=15) control group, received placebo infusion of saline, Group II (n=15) received TA; before skin incision (15mg/kg) and after CPB (15mg/kg) bolus, Group III (n=15) received a bolus dose of TA (15mg/kg) followed by a lmg/kg infusion for 6 hr. Recorded parameters were; ACT, haemoglobin (Hb), haematocrit (Htc), platelet count, volume and Htc of shed blood through the drains. aPTT, PT, flbrinogen, and transfusion requirements. Data were recorded; after protamine administration and at postoperative 6, 12 and 24 hr. Results: ACT was higher in group II than HI at 6 hr postoperatively (p<0.05). Hb level was higher in Group III than I at postoperative 24 hr (p<0.05). Shed blood through the drains in 24 hr was less in Groups II (p<0.05) and III (p<0.01) than I. Fibrinogen levels were lower after CPB and at 6, 24 hr postoperative in Group III than I (p<0.05), but the mean values were within the normal laboratory range. Mean units of blood and fresh frozen plasma transfusion was less in Group III than I and II (p<0.05). Conclusion: Although bolus administration of TA is effective in decreasing bleeding compared to placebo,continuous infusion regimen is superior in decreasing blood loss and reducing the requirement for replacement therapy.

Amaç: Kardiyopulmoner bypass (KPB) uygulanan hastalarda traneksamik asitin (TA) kanamayı azalttığı saptanmış, ancak ideal uygulama şekli ve dozu henüz kesin olarak belirlenmemiştir. Bu randomize kontrollü çalışmada TA infüzyonu ile bolus uygulaması postoperatif kanama, transfüzyon ihtiyacı ve koagulasyon parametrelerine etkileri açısından karşılaştırıldı. Yöntem: Elektif koroner arter cerrahisi planlanan 45 erişkin hasta çalışma grubuna alındı. Grup I (n=15) kontrol grubuna plasebo salin infüzyonu uygulandı. Grup II (n=15) 'deki hastalara cilt insizyonundan önce ve KPB sonunda iki kez bolus doz şeklinde 15 mg/kg TA uygulandı. Grup III'deki (n=15) hastalara ise cilt insizyonu öncesinde uygulanan 15 mg/kg bolus TA 'i takiben 6 saat süre ile l mg/kg/ saat hızda infüzyon uygulandı. Çalışma süresince; protamin uygulamasını takiben ve postoperatif 6, 12 ve 24. saatte ölçülen ve kaydedilen parametreler; ACT, hemoglobin (Hb), hematokrit (Htc), trombosit sayısı, direnlerden biriken kanın hacmi ve hematokriti, aPTT, PT, flbrinojen ve transfüzyon miktarı idi. Bulgular: Postoperatif 6. saatte ACT Grup II'de III'den daha yüksek bulundu (p<0.05). Postoperatif 24. saatte Hb Grup III'de Grup I'den daha yüksek idi (p<0.05). 24 saatlik direnaj miktarı Grup II (p<0.05)ve III'de (p<0.01) Grup I'den daha az idi. Fibrinojen düzeyi protamin uygulamasını takiben ve postoperatif 6 ve 24. saatte Grup III'de I'den daha düşük bulundu (p<0.05) ancak ortalama değerler normal sınırlar içerisinde idi. Ortalama kan ve taze donmuş plazma transfüzyon miktarı Grup IIl'de Grup I ve II'den daha az idi (p<0.05). Sonuç: Bu çalışmada bolus TA uygulamasının kontrol grubu ile karşılaştırıldığında kanama miktarını azalttığı saptanmıştır, ancak TA infüzyonunun postoperartif kanama miktarının ve kan transfüzyonu ihtiyacının azaltılmasında hem kontrol hem de bolus uygulamadan daha etkili olduğu bulunmuştur.
