Hypothyroidism as a cause of colonic pseudo-obstruction

Abstract

Sixty eight-years-old woman who has been known as hypothyroid, was admitted to the hospital because of acute intestinal obstuction. Although clinical and radiological signs of mechanical colonie obstruction were existed, at laparatomy only excessive dilated colonie segments could be found. She was treated with peroperative colonie decompression by using rectal tube and no colonie diversion has been applied. The contrast enema of the colon was also normal in the postoperative period. For that reason the case was accepted as an acute colonie pseudo-obstruction due to hypothyroidsm. The dosage of thyroid hormone replacement was increased postoperatively. Patient recovered without any problem.

Daha önceden hipotiroidi olduğu bilinen 68 yaşında kadın hasta, akut intestinal obstrüksiyon nedeniyle hastaneye yatırıldı. Klinik ve radyolojik olarak mekanik kolon obstrüksiyonu bulguları olmasına karşın, laparatomide aşırı dilate kolon segmentleri dışında, obstrüksiyona neden olabilecek bir patoloji bulunamadı. Rektal tüp yardımıyla peroperatif kolon dekompresyonu yapılarak tedavi edilen hastaya herhangi bir kolonik diversiyon işlemi uygulanmadı. Postoperatif dönemde çekilen kontrastlı kolon grafısi de normal olarak bulunan hasta, hipotiroidiye bağlı akut kolonik pseudo-obstrüksiyon olarak değerlendirildi. Postoperatif tiroid hormon replasman dozu arttır ilan hasta sorunsuz iyileşti.
