Abstract

AIM: In this study; The fetal and maternal complications of 272 single pregnancies with macrosomic fetuses from 01.12.1996 to 28.02.1997 at Aegean Maternity Hospital were evaluated. MATERIAL AND METHOD: This study was performed at Aegean Maternity Hospital. 4000 gr. was the threshold for the macrosomic neonates. We evaluated that fetal weight is effected by parity, age, maternal weight, postmaturity and diabetes meltitus. RESULT: At three months time the number of newborns were 3394 and numbers of macrosomic deliveries were 272(8.01%). Diabetes or the history of diabetes was observed in 9.55% of the pregnancies with macrosomic fetuses Mean weight gain were 15.83±3.56kg. during the pregnancies. Average surterm was 11. 16±5.58 day at the 22.42% of the cases. Cesarean operation rate was 30.51% at the macrosomic group and 22.35% at the control group. Maternal and fetal complications and cesarean operation rates were significantly higher than macrosomic group. CONCLUSIONS: Maternal and fetal complication were observed together in macrosomic deliveries and so cesarean section is more important at risk groups.

AMAÇ: Bu çalışmada amaç, SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi Obstetri Departmanında 1.12.1996-28.2.1997 arasında 4000 gr. ve üzeri 272 tekil gebelik olgusu fetal ve maternal komplikasyonlar açısından değerlendirmektir. GEREÇ VE YÖNTEM: Araştırma SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi Obstetri departmanında yapıldı. Makrosomik yenidoğan için 4000 gr. eşik değer kabul edildi. Parite, yaş. maternal ağırlık. postmatürite ve diabetes mellitusun doğum ağırlığına etkileri değerlendirildi. BULGULAR: Üç aylık dönemde canlı doğum sayısı 3394, makrosomik doğum sayısı 272(%8.01) idi. Makrosomik doğum yapanlarda diabet varlığı ya da öyküsü %9.55 olarak saptandı. Gebelik boyunca alınan ortalama kilo 15.83±3.56 kg. idi. Olguların %22.42 sinde ortalama 11.16±5.58 gün, gün aşımı bulunmakta idi. Sezeryan doğum oranı, makrosomik grupta %30.51 iken, kontrol grubunda %22.35 idi. Operatif doğum maternal ve fetal diğer komplikasyonlar makrosomik grupta kontrol grubundan belirgin olara yüksek bulundu. SONUÇ: Makrosomik doğum maternal ve fetal komplikasyonu beraberin de getirmektedir. Bu nedenle risk gruplarında sezeryan ile doğum daha önem kazanmaktadır.
