Hepatitis B prevalance in children with type 1 diabetes mellitus and response to hepatitis B vaccine

Abstract

OBJECTIVE: To investigate the prevalence of hepatitis B and immunresponse to hepatitis B vaccine in the children with type 1 diabetes mellitus. METHODS: The seropositivity of hepatitis B surface antigen (HBs Ag), and antibodies against surface and core antigens (anti-HBs and anti-HBc) were investigated with ELISA in 150 children with diabetes mellitus and in 100 healthy children. In addition, immunresponses to hepatitis B vaccine were investigated in 37 children with diabetes and in 49 healthy children, who had negative hepatitis B markers at initiation and immunised with 3 doses of recombinant hepatitis B vaccine. RESULTS: HBs Ag, anti-HBS, and anti-HBc seropositivity in the children with diabetes were found 3.3%, 8%, 11.3% respectively. In healthy children these results were 3%, 7%, and 10% and there were no significant differences between the groups (p>0.05). The immunresponse to hepatitis B vaccine was achieved in all of the children. However, the mean level of and HBs titer was 992.5±543.3 mIU/ml in diabetic children, and 1632±899. 7 mIU/ml in healthy children; and the level of anti HBs titer in diabetic children was significantly lower than the healthy group (p<0.05). CONCLUSION: Seropositivity of HBs Ag and the other hepatitis B markers was not higher in type 1 diabetic children than healthy children, but antibody titer to hepatitis B vaccine was lower. It was thought that this difference might be due to genetic factors and immunologic function abnormalities in children with diabetes mellitus.

AMAÇ: Tip 1 diabetes mellituslu çocuklarda hepatit B prevalansını ve hepatit B aşısına karşı yanıtı araştırmak. YÖNTEM: Yüzelli diabetli ve 100 sağlıklı çocukta ELİSA yöntemi ile hepatit B virusu yüzey antijeni (HBsAg) seropozitifliği ile hepatit B virus yüzey ve kor antijenine karşı geliştirilen antikorların (anti-HBs ve anti-HBc) varlığı araştırıldı. Ayrıca bu markerler negatif bulunan ve üç doz rekombinant hepatit B aşısı ile aşılanan 37 diabetli olguda ve 49 sağlıklı çocukda aşıya karşı yanıt incelendi. BULGULAR: Diabetli olgularda HBsAg taşıyıcılığı %3.3, anti-HBs pozitifliği %8, anti-HBc pozitifliği %11.3 idi. Sağlıklı çocuklarda ise bu sonuçlar sırası ile %3, %7 ve %10 idi ve iki grup arasında anlamlı bir fark yoklu (p>0.05). Aşıya karşı cevap her iki gruptaki olguların tümünde oluştu. Ancak ortalama anti-HBs türesi diabetli olgularda 992.5±543.3 mIU/ml, kontrol grubunda ise 1632±899.7 mIU/ml idi ve diabetli olgulardaki titre kontrol grubuna göre anlamlı şekilde düşüktü (p<0.05). SONUÇ: Tip 1 diabetli olgularda HBsAg taşıyıcılığı ve diğer hepatit B markerleri normal populasyona göre yüksek bulunmadı; ancak aşıya karşı antikor türesi diabetli olgularda daha düşük bulundu. Bu farklılığın diabetli olgularda immun sistemin fonksiyonlarındaki bozukluklar ve genetik özellikler nedeniyle oluşmuş olabileceği düşünüldü.
