Case report: Giant hyperthyroid goiter with amyloidosis requiring urgent thyroidectomy

Abstract

Amyloidosis is a disease resulting from accumulation of fibril like proteins in organs. Amyloidal goiter is a component of systemic amyloidosis which is rarely seen. Microscopic amyloid accumulation is generally seen in thyroid tissue, however thyroid rarely reaches giant size with amyloid accumulation and amyloid accumulation rarely causes hyper functioning of thyroid gland. In this case report, we presented the hyperthyroid giant amyloidal goiter following the renal transplantation. The patient's thyroid gland had rapidly reached the giant size in a short time and it caused acute respiratory failure requiring urgent operation.

Özet

Amiloidozis fibril yapısı gösteren proteinlerin doku ve organlarda birikmesidir. Amiloid guatr sistemik amiloidozisin nadir görülen bir komponentidir. Tiroid bezinde çoğunlukla mikroskopik birikim gözlenir, nadiren dev boyutlara ulaşır ve tiroid fonksiyon testlerinde bozulmaya sebep olur. Biz de renal transplantasyon sonrası gelişen dev amiloid guatr olgusunu sunmayı amaçladık. Olgumuz çok kısa zamanda çok büyük boyutlara ulaşarak akut solunum sıkıntısına yol açan, beklenenin aksine hipertiroidi ile seyreden amiloid guatr olgusudur.
