Mammographic changes in postmenopausal women under hormone replacement therapy (HRT)

Abstract
PURPOSE: In this study, it is aimed to detect the frequency, distribution and degree of density changes in the breast parenchyme in women under hormone replacement therapy (HRT) and to investigate whether there is any difference between different therapy regimens. MATERIAL AND METHODS: Of the 66 postmenopausal women included into this study, 23 were taking estrogen alone (Group A) and 43 estrogen in combination with progesterone (Group B). Mammograms obtained before and after the initiation of therapy were evaluated for density changes. In addition, sonograms obtained in some patients for the evaluation solid or cystic masses were reviewed. RESULTS: Density increase was detected in 5 cases (%22) in Group A and in 13 cases (%30) in group B. Density increase was mostly diffuse (17%) in distribution and mild (61%) in degree. The frequency of density increase was 13% (3/26), 47% (9/19) and 29% (6/21) in cases with a follow-up period of 4-12 months, 12-24 months and longer than 24 months respectively. CONCLUSIONS: Although not statistically significant density increase is more common in cases under combined HRT when compared with the cases taking estrogen alone. Our findings show that the duration of follow-up is related with the number of cases with density increase.

AMAÇ: Bu çalışmada postmenopozal hormon replasman tedavisi (HRT) uygulanan kadınlarda meme parankiminde oluşan dansite değişikliklerinin sıklığının, dağılım şeklinin ve derecesinin saptanması ve farklı tedavi tipleri arasında bu değişikliklerin özellikleri yönünden farklılık bulunup bulunmadığı araştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Bu retrospektif çalışmaya 23'ü tek başına östrojen (Grup A), 43'ü kombine östrojen ve progesteron (Grup B) olmak üzere HRT alan 66 postmenopozal kadın dahil edilmiştir. İzlem süresi Grup A'da 20.12±12.70 ay, Grup B'de 18.79±12.60 ay olarak hesaplanmıştır. Olguların HRT öncesinde ve tedavi başlanması sonrasında alınan mamografileri dansite değişiklikleri yönünden incelenmiştir. Ek olarak bazı olgularda solid veya kistik kitle patolojilerinin değerlendirilmesine yönelik olarak yapılmış olan US incelemeleri gözden geçirilmiştir. BULGULAR: Grup A'da 5 olguda (%22) dansite artışı görülürken Grup B'de 13 olguda (%30) dansite artışı saptanmıştır. Dansite artışı en çok (%17) difüz dağılımlı ve hafif derecede (%61) görülmüştür. Dansite artış oranı izlem süresi 4-12 ay, 12-24 ay ve 24 aydan uzun olan olgularda ayrı ayrı %13 (3/26), %47 (9/19) ve %29 (6/21) olarak hesaplanmıştır. SONUÇ: Kombine HRT alan olgularda tek başına östrojen alanlarla karşılaştırıldığında istatistiksel olarak anlamlı olmamakla birlikte dansite artışı daha sık görülmektedir. Bulgularımız izlem süresi ile dansite artışı meydana gelen olgu sayısının ilişkili olduğunu göstermektedir.
