A comparison of fetomaternal and neonatal outcome between gestational and pregestational diabetes mellitus

Abstract
OBJECTIVE: To determine the incidence of antenatal and intrapartum maternal and fetal complications of gestational diabetes mellitus (GDM) and compare them with pregestational diabetes mellitus (pre-GDM). MATERIAL AND METHODS: 183 women with gestational diabetes mellitus and 39 women with pre-gestational diabetes mellitus, and their offspring (delivered in our hospital between January 1998 and December 2000) were included to our study. Maternal and fetal complications seen during prenatal, intrapartum and neonatal periods were analyzed. RESULTS: The high incidence of maternal complications including preterm labor, pregnancy induced hypertension, polyhydramnios, cesarean section were found in both GDM and pre-GDM patients. Also high rates of macrosomia and hypoglycemic episodes were observed in their offspring. CONCLUSION: The incidence of maternal, fetal and neonatal complications in GDM is similar to pre-GDM patients and their offspring.Also there is no difference in their follow up and management.

AMAÇ: Gestasyonel diabetes mellitus (GDM) olgularındaki antenatal ve intrapartum maternal ve fetal komplikasyonların insidansının tesbit etmek be bu sonuçları pregestasyonel diabetes mellitus(pre-GDM) olgularından elde edilen sonuçlarla karşılaştırmak. GEREÇ VE YÖNTEM: Ocak 1998- Aralık 2000 tarihleri arasında hastanemizde takip edilen ve doğumları gerçekleştirilen 183 GDM'li ve 39 pre-GDM'li gebe ve onların yenidoğanları çalışma kapsamına alındı. Prenatal, intrapartum ve neonatal dönemde gözlenen maternal ve fetal komplikasyonlar analiz edildi. BULGULAR: Preterm eylem, gebeliğin indüklediği hipertansiyon, polihidramnios, sezaryen gibi maternal komplikasyonların hem GDM hem de pre-GDM olgularında yüksek oranda olduğu saptandı. Aynı zamanda bu gebelerin yenidoğanlarında yüksek makrozomi ve hipoglisemi oranları tesbit edildi. SONUÇ: GDM'li hastalarda gözlenen maternal, fetal ve neonatal komplikasyon oranları pre-GDM'li hastalar ve yenidoğanlarında saptanan oranlarla benzerdir. Bu hastaların takip ve tedavileri de birbirleri ile benzerlik göstermektedir.
