Ceftriaxone induced pseudocholelithiasis in two cases

Abstract
A ten month old male infant was diagnosed as acute otitis media and acute gastroenteritis and was receiving sulbactam-ampicillin therapy. Ceftriaxone was administered because of his urinary tract infection due to positive urine culture. On the third day of treatment abdominal ultrasonography revealed gallstones that fills one fifth of the gallbladder and a renal stone that's 2.5 mm. in diameter in the renal calyx. Control abdominal ultrasonography, that's performed on 10th day of treatment showed microstones in the gallbladder. No renal stones were detected in the control ultrasonography. A third control ultasonography that's performed one month after the completion of therapy revealed complete disappearance of gallstones. The investigations relating to the origin of the gallstones and renal stone failed to obtain any etiologic reason for the stone formation. We thought that the ceftriaxone therapy was the cause of stone formation. Second case was a 3-year-old male toddler who was receiving ceftriaxone and sulbactam-ampicillin therapy, to whom on abdominal ultrasonography was performed due to abdominal pain that revealed multiple small stones in the gallbladder. The investigations relating to the possible pathologies for gallstone formation suggested that, those stones could be secondary to ceftriaxone therapy. On his control abdominal ultrasonography the stones in gallbladder were disappeared. All posible pathologies for the stone formation were studied in both cases, but no etiologic reason could be determined. We come to conclusion that in both cases the stone formation was due to ceftriaxone therapy. The gallstones secondary to ceftriaxone therapy are reversable irrelevant of the dose and the interval of the therapy. They are named as pseudocholelithiasis, hereby, we present our two cases of pseudocholelithiasis secondary to ceftriaxone therapy.

Akut gastroenterit, akut otitis media tanılarıyla izlenen sulbactam-ampisilin tedavisi alan 10 aylık erkek olguya, alınan idrar kültüründe üreme olması üzerine idrar yolu enfeksiyonu için seftriakson başlandı. Tedavinin 3.gününde çekilen batın ultasonografisinde safra kesesinde kesenin 1/5'ini dolduran safra kesesi taşları ve böbrek kaliksinde 2.5 mm çapında böbrek taşı saptandı. Tedavinin 10.gününde çekilen kontrol ultasonografisinde ise safra kesesinde grup oluşturan mikro taşlar görüldü, ancak böbrek taşı kayboldu. Tedavi bitiminden bir ay sonra çekilen kontrol ultrasonografisinde safra kesesi taşları da kaybolmuştu. Safra kesesi ve böbrek kaliksinde oluşan taşlar için yapılan tetkiklerde, taşların oluşumunu açıklayacak etyolojik neden bulunamadı. Bu taşların, hastanın tedavisi için kullanılan seftriaksona bağlı olduğu düşünüldü. İkinci olgu ise; pürülan menengit nedeniyle seftriakson ve sulbactam-ampisilin alan ve karın ağrıları olması üzerine çekilen batın ultrasonografisinde safra kesesi içinde taşlar saptanan 3 yaşında erkek hasta idi. Yapılan tetkiklerde safra kesesinde taş oluşturacak patolojinin, hastaya tedavisi için kullanılan seftriakson olduğu anlaşıldı. Yapılan kontrol ultasonografisinde safra kesesi taşları kaybolmuştu. Her iki olguda safra kesesi taşlarının nedeni olabilecek olası tüm patolojik nedenler araştırıldı, ancak etyolojik neden bulunamadı. Safra kesesi ve böbrek taşlarının, hastalarda kullanılan seftriaksona bağlı olduğu düşünüldü. Seftriaksona bağlı safra kesesi taşları; doza, süreye bağlı olmaksızın oluşan ve reversibl olan, psödokolelitiasis olarak isimlendirilen taşlardır. Seftriaksona bağlı iki psödokolelitiasis olgumuzu, ilginç olması nedeniyle sunmayı düşündük.
