The effects of different doses of oxytocin and salin solution used during labor to neonatal jaundice

Abstract
Objective: In this study we aimed to investigate that the effects of both low dose and high dose oxytocin together with different amounts of saline solution used during labor to neonatal jaundice. Material and Method: Study was conducted on 157 pregnants who were administered oxytocin and 50 controls and those of full-term newborns between January 2000 and August 2001. Fifty-three newborns (group 1) whose mother had low dose (<1500 µU) oxytocin plus saline solution (<250 ml) and 58 newborns (group 2) whose mother had high dose (>1500 µU) oxytocin plus saline solution (>250 ml) and 50 newborns whose mother had no drug or fluid (Controls) could complete the study. Results: Whereas, gestational age, birth weight, gender distribution, Apgar scores, and cord blood hemoglobin, hematocrit, total bilirubin values were not different, total bilirubin concentrations in 48th, 72nd and 96th hours of the newborns in group 2 were significantly higher and hematocrit counts were significantly lower than newborns those of group 1 and controls (p<0.05). We found that a positive and significant correlation between oxytocin doses used during labor and total bilirubin concentrations of newborns in 48th, 72nd and 96th hours of their life (r= 0.3 p=0.01, r=0.57 p=0.01, r=0.61 p=0.01, respectively). Conclusion: As a result, we found that low dose oxytocin plus saline solution using for labor may decrease the risk of neonatal jaundice. However, when high dose oxytocin plus fluid are used during labor, newborns should be followed closely for increased risk of hyperbilirubinemia.

Amaç: Bu çalışmada doğum eylemi için düşük ve yüksek dozda oksitosin ile birlikte farklı miktarlardaki salin solüsyonu kullanımının yenidoğan sarılığına etkilerini araştırmak amaçlandı. Gereç ve Yöntem: Çalışma Ocak 2000-Ağustos 2001 tarihleri arasında doğum eylemi sırasında oksitosin uygulanan 157 ve kontrol grubu olarak seçilen 50 anne ve miadında yenidoğanlarında gerçekleştirildi. Annelerine düşük doz oksitosin ( 1500 µU) ve salin solüsyonu (> 250 ml) uygulanan 58 yenidoğan (2. grup) ve annelerine ilaç ve sıvı uygulanmayan 50 yenidoğan (kontrol) çalışmayı tamamlayabildi. Bulgular: Çalışma grubu ve kontrol grubundaki bebeklerin gestasyon yaşı, doğum ağırlığı, cinsiyet dağılımı, Apgar skorları, kordon kanı hemoglobin, hematokrit ve total bilirubin değerleri birbirine benzerdi. Doğum eylemi sırasında yüksek doz oksitosin uygulanan 2. gruptaki bebeklerin 48, 72 ve 96. saatlerdeki total bilirubin değerleri düşük doz oksitosin ve sıvı verilen 1. gruptaki bebeklerden ve kontrol grubundan anlamlı yüksek, hematokrit değerleri ise anlamlı düşük bulundu (p< 0.05). Doğum eylemi sırasında uygulanan oksitosin dozları ile, yenidoğanların doğumdan sonra 48, 72 ve 96. saatlerdeki total bilirubin değerleri arasında pozitif doğrusal ve anlamlı bir korelasyon bulundu (sırasıyla r= 0.3 p=0.01, r=0.57 p=0.01, r=0.61 p=0.01). Sonuç: Çalışmanın sonucuna göre doğum eyleminde düşük doz oksitosin ve salin solüsyonu kullanımı ile neonatal sarılık riskinin azaltılabileceği bulunmuştur. Bununla birlikte, yüksek doz oksitosin ve sıvı uygulandığında ise yenidoğan bebekler yüksek hiperbilirubinemi riski açısından yakın izleme alınmalıdır.
