The neonatal seizures and cerebral venous sinus thrombosis

Abstract
Objective: The aim of this study was to evaluate the features of neonatal seizures and to emphasize importance of cerebral venous sinus thrombosis (CVST) in this clinical entity. Material and Method: Fifty-two mature newborn babies hospitalized for their seizures, were retrospectively evaluated as clinical and laboratory features and prognosis. All living cases had been followed for morbidity during one-year time. After the cases with CVST had been determined, the features of the cases with CVST (Group 1) and without CVST (Group 2) were compared, statistically. Results: The most frequent reason of seizures during postnatal first three days was perinatal asphyxia while it was septicaemia in seizures after postnatal three days. CVST was determined in 7 cases (13.5%). In our series, the major reasons of the CVST were perinatal asphyxia, septicemia and polycythemia. Refractory and focal clonic seizures were found to be more frequent in Group 1 than Group 2 (p<0.001). There was no significant difference for mortality and morbidity rates between groups. Conclusion: CVST must be investigated in the differential diagnosis of neonatal seizures, particularly in the cases with refractory and focal seizures.

Amaç: Bu çalışmanın amacı; konvülsiyonlu yenidoğanların özelliklerinin değerlendirilmesi ve serebral venöz sinüs tromboz (SVST)'unun bu klinik durumdaki önemininin vurgulanmasıdır. Gereç ve Yöntem: Yenidoğan ünitemizde konvülsiyon nedeniyle yatırılan 52 term bebek klinik ve laboratuvar özellikler ve prognoz açısından retrospektif olarak değerlendirildi. Tüm olgular morbidite yönünden bir yıl süreyle takip edildi. Çalışma grubundaki SVST'li olgular belirlendikten sonra, SVST'li olgular (Grup 1) ile SVST olmayan olguların (Grup 2) özellikleri istatistiksel olarak karşılaştırıldı. Bulgular: Postnatal ilk üç günde konvülsiyon geçirenlerde en sık neden perinatal asfiksi iken, üçüncü günden sonra en sık neden sepsis idi. SVST, 7 olguda (%13.5) saptandı. SVST'nin sebepleri, sırasıyla; perinatal asfiksi, sepsis ve polisitemi olarak bulundu. İnatçı ve fokal klonik nöbetler Grup 1'deki olgularda Grup 2'deki olgulardan daha fazla gözlendi. Mortalite ve morbidite oranı açısından gruplar arasında anlamlı farklılık saptanmadı. Sonuç: Yenidoğan konvülsiyonlarının ayırıcı tanısında; özellikle inatçı ve fokal konvülsif nöbetler geçiren hastalarda SVST'nin de araştırılması gerekir.
