Comparison of efficacy of diet and low-dose statin therapy in patients with hyperlipidemia

Abstract
In the present study efficacy of diet, low pravastatin, simvastatin, fluvastatin on lipid fractions were compared in 121 hyperlipidemic patients who applied to our lipid and hypertension clinic. Subsequent to a two month diet therapy directed to diminish lipid in all patients, four randomised groups were formed. There was not any statistically significant difference among the groups regarding age, sex, hypertension, smoking, diabetes mellitus, familial coronary disease, early menopause, previous revascularisation and lipid parameters (Total cholesterol, HDL cholesterol, triglyceride, LDL cholesterol, apolipoprotein Al, apolipoprotein B). 29 patients in the first group (Group I) were given pravastatin 10 mg/day/po, 29 patients in the second group(Group II) were given simvastatin 10 mg/day/po and 6 patients in the third group (group III) received fluvastatin 40 mg/day/po, 57 patients in the fourth group (Group IV) received only antilipidemic diet therapy. All of the patients were followed up for six months. ANOVA test was used in groups in the study. While there was a significant decrease in triglyceride values and first second, and fourth groups total cholesterol values in all the groups compared with the values before treatment, an increase in apolipoprotein AI and a decrease in apolipoprotein B was not significant. Decreases in LDL cholesterol in Group I and Group II were significant. Although increases in HDL cholesterol level were present in all groups, they were significant only in Group I. It was established that there was not a significant difference as to their antilipidemic effects in diet and drugs in statin groups and in only drugs in statin groups for period of 6 months and the given doses. As a result, we came to the conclusion that low - dose statin therapy is not superior to diet therapy alone

Bu çalışmada lipid ye hipertansiyon polikliniğine müracaat eden 121 hiperlipidemik hastada diyet, düşük doz pravastatin, simvastatin, fluvastatin'in lipid fraksiyonları üzerine etkinliği karşılaştırıldı. Tüm hastalara 2 aylık lipid düşürücü diyet tedavisi uygulandıktan sonra randomize 4 grup oluşturuldu. Gruplar arasında yaş, cins, hipertansiyon, sigara, D.Mellitus, ailede koroner hastalık bulunması, erken menopoz, revaskülarizasyon geçirmiş olma ve lipid parametreleri (total kolesterol, HDL kolesterol, trigliserid, LDL kolesterol, apolipoprotein Al, apolipoprotein B) yönünden istatistiksel anlamlı farklılık yoktu. I.Gruptaki 29 hastaya Pravastatin 10 mg/gün/p.o., II.Gruptaki 29 hastaya Simvastatin 10 mg/gün/p.o., III. Gruptaki 6 hastaya fluvastatin 40 mg/gün/p.o., IV. Gruptaki 27 hastaya sadece antilipidemik diyet tedavisi uygulandı. Tüm hastalar ortalama 6 ay takip edildi. Çalışmada gruplar arasında ANOVA test (tekyönlü variyans analizi) kullanıldı. Tüm gruplarda tedavi ile trigliserid ve I. II. ve IV. Gruplarda total kolesterol değerlerinde tedavi öncesine göre anlamlı düşüş olurken, apolipoprotein Al'deki yükselme ile apolipoprotein B'deki azalma anlamlı bulunmamıştır. LDL kolesteroldeki azalmalar I ve II. gruplarda anlamlı, HDL kolesterol seviyesinde yükselmeler tüm gruplarda olmasına rağmen, sadece I. Grupta anlamlı bulunmuştur. 6 aylık sürede ve belirtilen dozlarda diyet ve statin grubu ilaçlar ile statin grubu ilaçların kendi aralarında antilipidemik etkileri yönünden istatistiksel analizde fark olmadığı saptandı. Sonuç olarak hiperlipidemik hastalarda düşük doz statin tedavilerinin tek başına diyet tedavisine üstünlükleri olmadığı kanısına varıldı.
