Asendan aort anevrizmalı hastalarda uyguladığımız cerrahi yöntemler

Abstract

We present an experience with 12 patients, underwent surgical teratment for aneurysm of the ascending aorta between June, 28, 1993-May, 14, 1995 in our clinic and we reviewed the results of 12 patients. Marfan's syndrome was present in 8 and atherosclerotic aneurysm of the ascending aorta was present in 4 patients. While the 6 of patients with Marfan's syndrome underwent Bentall procedure, 2 of them had Cabrol procedure also had MVR. Supracoronary graft interposition procedure were achieved in patients with atheroslerotic aneurysm of the ascending aorta and CABG were added in those 2, who had also coronary artery stenosis. lof 12 patients died of low cardiac Output and multiple systems failure in postoperative 7th day. 1 patient was reoperated in order to control of bleeding. Our follow-up time is 164 months, none öf the other patients showed mortality and morbidity. In the results of the postoperative sixth months reveal, in the functional capacities of the patients significant elevations have been found compared to the postoperative values. Conclusion; These results suggest that in aneurysm of the ascending aorta, surgical approach is the only and the most effective way of management.

Özet

Kliniğimizde 28 Haziran 1993-14 Mayıs 1995 tarihleri arasında 12 hastaya asendan aort anevrizması tanısıyla cerrahi tedavi uygulanmış olup bu hastaların sonuçlan değerlendirilmiştir. Bu hastaların 8'i Marfan Sendromu, 4'ü atherosklerotik asendan aort anevrizması tanılar almıştır. Marfan Sendromlu hastalardan 6'sına Bentall ameliyatı, 2'sine de Cabrol prosedürü uygulanmıştır. Bentall ameliyatı uygulanan hastalardan ikisine mitral kapak replasmanı cerrahi girişime ilave edilmiştir. Atherosklerotik asendan aort anevrizması olan hastalara suprakoroner graft interpozisyon ameliyat uygulanmış, bu hastalardan ikisine stenotik koröner arterlerinden dolay koroner arter bypass grafting ameliyat cerrahi prosedüre eklenmiştir. 1 hastamız postoperatif 7. günde düşük kalp debisi ve multiple organ yetmezliği nedeniyle exitus olmuştur. 1 hastamız da kanama nedeniyle reoperasyona alınmıştır. Toplam takip süremiz 164 aydır. Diğer hastalarımızda mortalite ve morbiditeye rastlanılmamıştır. Postoperatif 6. ayda yapılan değerlendirme sonucunda hastaların preoperatif değerlerine göre fonksiyonel kapasitelerinde belirgin artış olduğu görülmüştür. Asendan aort anevrizmasında cerrahi tedavi tek ve seçkin bir yöntemdir.
