The role of sex hormone in the development of hepatocellular carcinoma in cirrhotic patients

Abstract
To investigate the possible role of'sex-horpione in hepatocellular carcinogenesis in male hepatic cirrhotic patients, we determined plasma levels of testosterone, dehydroepiandrosterone (DHEA-S), estradiol in 15 men with hepatic cirrhosis alone and in 15 similar men with hepatic cirrhosis and hepatocellular carcinoma (HCC). Also 15 healthy men were included to the study as a control group. There were no difference between the mean ages and severity of liver disease of cirrhotic groups with HCC or without HCC. Patients of both groups had decreased plasma testosteron and DHEA-S levels (p<0.001) and hyperestrogenemia with an increase in estradiol (p<0.001) plasma levels compared with 15 healthy age-matched controls. Cirrhotic patients with HCC had lower concentrations of testosteron and DHEA-S than patients with cirrhosis alone; but the differences were not significant statistically (p>0.05). These results suggest a possible promotive role for oestrogens in the development of HCC from the cirrhotic liver.

Hepatik siroz gelişmiş erkek hastalarda sex-hormonlarının hepatosellüler karsinogenesisindeki rolünü saptamak amacıyla hepatik sirozu olan 15 erkek hastada ve hepatosellüler karsinom (HCC) gelişmiş benzer 15 erkek hastada plasma testosterone, dehidroepiandrosterone (DHEA-S), estradiol düzeyleri araştırıldı. Ayrıca çalışmaya kontrol grubu olarak 15 sağlıklı erkek alındı. Hepatosellüler karsinom ile beraber olan veya olmayan karaciğer sirozlu hasta grupları arasında yaş ortalaması ve karaciğer hastalığının şiddeti yönünden farklılık bulunmamaktada idi. Her iki gurup hastada plasma testosteron ve DHEA-S düzeylerinde sağlıklı kontrol grubuna oranla anlamlı azalma (p 0.05). Bu bulgular östrojenin, sirotik karaciğer hastalarında HCC gelişmesinde destekleyici rol oynadığını ortaya koymaktadır.
