Analgesic action of metoklopramid in postthoracotomy pain

Abstract

Besides being used for antiemesis metoklopramid has also been used as an analgesic in labor, urolithiasis, hip surgery and migraine. The aim of this study is to evaluate the effectiveness of metoklopramid infusion in posthtoracotomy pain. This study was planned as double - blind research. After being informed, the patients schecluded for thoracotomy operation vere divided into two groups. The first (M) (n=19) and second (K) (n=16) groups were given 0.5 mg/kg of metoklopramid in 100 ml isotonic Nad solution and 100 ml isotonic NaCl solution respectively in 15 min before anaesthesia induction. Induction and maintenance of anaesthesia was achieved with 7 mg/kg thiopental, 0.1 mg/kg vecuronium, 1.5% isoflurane in 50% O2 + 50% N2O in all groups. The first group received 2 mg/kg of metoklopramid in 200 ml of isotonic NaCl solution and the second group 200 ml of isotonic NaCl solution for 4hours during surgical intervation. Additional analgesic or sedatives were not given to either group during the operation. Patients were evaluated postoperatively by physicians and nurses who did not know the groups of patients.Patients were asked to determine the exact onset of pain. The severity of pain was evaluated using visual analog scores (VAS) and verbal pain scores (VPS) and if VAS35 and VPS33, 25 mg meperidine was injected intramuscularly. The VAS and VPS scores were evaluated at 30, 60, 90, 120, 150, 180, 210, 240 min and at 12 and 24 h after operation. The urine output, side effects, total doses of meperidine were recorded after 24 h.The VAS and VPS scores were significantly high at 30 and 60 min postoperative in the control group (p<0.05). M group was found to be better in the evaluation of patient, physician and nurse for 24 h (p<0.05). The urine output was less in M group and nausea was milder (p<0.05). The time lapse to begin treatment for pain with meperidine was significantly longer in group M (p<0.05) and the total dose of meperidine in M group was also lower as compared to control group (p<0.05).The analgesic effect of metoklopramid and reduction of meperidine need was observed in postthoracotomy patient. Consequently, metoklopramid was found to be an effective analgesic in severe postthoracotomy pain, which was considered as severe pain with minimal side effects.

Metoklopramitin antiemetik özelliğinin yanısıra doğumda, ürolitiazisde, kalça cerrahisinde, akut migren ağrılarında analjezik özelliği olduğu bildirilmiştir. Bu çalışmanın amacı; infüzyon şeklinde uygulanan metoklopramitin posttorakotomi ağrısında etkisini saptamaktır. Çalışma öncesi torakotomi geçirecek hastalar işlem hakkında bilgilendirildikten sonra rastgele iki gruba ayrıldılar, l gruba (n- 19) (metoklopramit =M grubu) anestezi indüksiyonundan önce 100 ml izotonik içinde 0.5 mg/kg metoklopramit konup 15 dakikada infüzyon yapıldı. Il.grup (kontrol =K grubu) 100ml izotonik 15 dakikada infüze edildi.Anestezi indüksiyonu ve idamesi sırasıyla; 7 mg/kg tiopental, 0.1 mg/kg vekuronyum %1-1.5 isofloran + %50 02+ %50 N2O ile sağlandı. Cerrahi girişimle birlikte I.grupta 200 mi izotonik 4 saat içinde infüzyonu yapılırken, II. grupta 200 ml izotonik içinde 2 mg/kg metoklopramid konup 4 saat içinde infüzyonu yapıldı. Operasyon süresi içinde hiçbir hastaya ilave analjezik veya sédatif yapılmadı. P ostoperatif dönemde yangi yöntemin kullanıldığını bilmeyen doktor ve hemşire tarafından hastalar izlendi. Hastaların ağrıları sorularak, ağrılarını VAS ( visual analog scala) ve VPS (verbal pain scor ) ile derecelendirmeleri istendi. Ağrı şiddeti VAS 35 ve VPS33 ise; 25 mg meperidin i.m uygulandı. Ağrı ölçümleri 30, 60, 90, 120, 150, 210, 240 dakikalarda 12 ve 24 saatlerde VAS ve VPS göre derecelendirildi. 24 saat sonunda ilk meperidin uygulama süresi, toplam meperidin dozu ile yöntemin etkinliğinin hasta, doktor ve hemşire tarafından değerlendirilmesi ve yan etkiler ile idrar miktarı kaydedildi. Hastaların VAS ve VPS değerlendirilmesinde K grubu değerleri, M grubuna göre 30 ve 60 dakikalarda istatistiksel olarak anlamlı derecede yüksekti (p<0.05 ). P ostoperatif dönemde meperidin ile ağrı tedavisine başlama zamanı M grubunda belirgin şekilde uzun bulunurken (p<0.05 ) aynı grupta 24 saatlik uygulanan toplam meperidin dozu daha azdı (p<0.05 ). Hasta, doktor ve hemşirenin 24 saatlik değerlendirmesinde M grubu daha iyi bulundu (p<0.05 ). M grubunda 24 saatlik toplam idrar miktarı daha düşüktü (p<0.05) ve bulantı daha azdı (p<0.05 ) Sonuç olarak; infüzyon şeklinde uygulanan metoklopramitin en şiddetli ağrı tiplerinden birisi olan torakotomi ağrısında meperidin dozunu azaltması bu ilacın minimal yan etkiyle etkin bir analjezi sağladığını kanıtlamaktadır.
