The prevalance of cholelithiasis in our patients with liver cirrhosis

Abstract
We retrospectively studied 92 cirrhotic patients hospitalized in the clinic of internal medicine, faculty of medicine, in region of Elazığ, between January 1992 and October 1996, aimed to determine the frequency of cholelithiasis and its main clinical and epidemiological features. Ultrasonography was performed for cholelithiasis in all patients. Twenty-eight out of 92 cirrhotic patients (% 30) had cholelithiasis. Cases were 64 males (mean age: 50.16±14.22 yr), 28 females (mean age :45.46±14.41).The prevalance of cholelithiasis according to sex was not significant. The prevalance of cholelithiasis was 26 of 78 (.33 % ) in decompansated cirrhosis and 2 of 14 (14 %) in compansated group were affected (p<0.05). According to the etiology of cirrhosis, hepatitis B was % 28, hepatitis C was % 27, alcoholic ve other were %43 in cirrhotic patients with cholelithiasis. The severity of liver disease, according to Child-Pugh classification influenced in the cholelithiasis frequency (p<0.05) and 42% of patients with gallstones were in grade C of Child Pugh Score. We conclude: Cholelithiasis in our cirrhotic patients more prevalent than normal population. The severty of liver disfunction influenced in highly significant way (p<0. 05) on cholelithiasis prevalance.

Ocak 1992 ile ekim 1996 tarihleri arasında İç Hastalıkları kliniğinde yatan Elazığ bölgesi karaciğer sirozlu olgularda retrospektif olarak safra kesesi taşı sıklığı; klinik ve epidemiyolojik özelliklerini araştırdık. Ultrasonografik olarak incelenen 92 hastanın 28'( %30) inde kolelityaz saptandı. Olguların 64' ü erkek (ortalama yaş:50.16±14.22 yıl), 28'i kadın (ortalama yaş: 45.46^14.41) idi ve cinsler arasında anlamlı fark yoktu (p>0.05). Dekompanze karaciğer sirozlu 78 olgunun 26' sında (%33), kompanze sirozlu 14 olgunun 2'sinde ( % 14) safra kesesi taşı tespit edildi (p<0.05) Sirozun Biyolojisine göre kolelityaz sıklığı: Hepatit B'de % 28, hepatit C'de % 27, alkolik ve diğer nedenlerde ise % 42'iydi. Child sınıflamasına göre sirozun derecesi ve safra kesesi taşı sıklığı arasında ilişki vardı (p<0.05) ve Child C grubu hastalarda % 42'iydi. Sonuçta: Kolelityaz sirotik hastalarımızda normal populasyona göre daha sık görülmektedir. Karaciğer sirozunun derecesi ile görülme sıklığı artmaktadır.
