The effects of nifedipine and captopril on proteinuria and renal functions in renal insufficiency

Abstract
The effect ofnifedipine which is a calcium channel blacker and captopril which is an angiotensin converting enyme inhibitor on renal functions were investigated in patients with chronic renal insufficiency. Both of the drugs are used the treatment of hypertension. 21 patients who attend to outpatient Internal Medicine Clinic at Trakya University Faculty of Medicine were included in the study group. The patients were of similar ages, had serum creatinine levels above 1,6 mg/dl and creatinine clearance level less than<80 ml/min/1,73 m2. None of the patients had oliguria. For three months 20-40 mg/day of nifedipine was administered to randomly selected 11 patients and 37,5-100 mg/day of captopril was given to 10 patients according to their creatinine clearance. The blood pressures, serum creatinines, creatinine clearance values, proteinuria levels and renal DMSA scintigraphies of the patients were compared.The mean arterial blood pressures in both groups were significantly decreased. However the creatinine clearance and the average DMSA uptake values were not statistically significant. In the nifedipine group there was an progressive increase in proteinuria. In the captopril group proteinuria values were decreased, but the reduction wasnot significant. Our findings suggest that in chronic renal failure patients, both of the drugs decrease the mean arterial blood pressure but nifedipine increases proteinuria and neither of the mean arterial blood pressure 'but nifedipine increases proteinuria and neither of the drugs effect glomerular filtration rate and perfusion.

Kronik böbrek yetersizliğinde (KBY), hipertansiyonun tedavisi için kullanılan kalsiyum kanal bloken nifedipin ve angiotensin konverting enzim inhibitörü kaptoprilin, böbrek fonksiyonlarına olan etkisi araştırıldı. Çalışmaya Trakya Üniversitesi Tıp Fakültesi İç Hastalıkları polikliniğine başvuran aynı yaş grubunda, serum kreatinini 1.6 mg/dl üzerinde kreatinin klirensi 80 ml/dk/1.73m2 altında olan, oligürisi olmayan çeşitli nedenlerden dolayı gelişmiş kronik böbrek hastalığı olan 21 hasta alındı. Rastgele olarak 11 hastaya 20-40 mg/gün nifedipin ve 10 hastaya kreatinin klirensine göre 37,5-100 mg/gün dozunda kaptopril 3 ay süre ile verildi, ilaçlar öncesi ve sonrası, kan basıncı değerleri, kreatinin, kreatinin klirensi, proteinüri ve kantitatif böbrek DMSA sintigrafileri karşılaştırıldı. Her iki grubta da ortalama arter basıncı anlamlı olarak düştü. Kreatinin klirensi ve DMSA uptake ortalama oranlarındaki değişiklikler anlamsızdı. Nifedipin grubunda proteinüri giderek arttı. Kaptopril grubunda ise proteinüri azaldı, ancak istatistiksel anlamlılık saptanmadı. Bulgularımız; KBY'de ortalama arter basıncını (OAB) her iki ilacın düşürmesine karşın, nifedipinin proteinüriyi arttırdığını, her iki ilacın da glomerüler filtrasyon ve perfüzyonu belirgin etkilemediğini düşündürmektedir.
