The functions of protein C and protein S and serum fibrinogen concentrations in obesity

Abstract

Both protein C and protein S are vitamin K-dependent proteins that are central to the natural anticoagulant pathway involving activated protein C. This pathway not only inhibits blood coagulation but also stimulates, flbrinolysis. Thrombosis is increasingly recognized as a central mechanism in stroke and myocardial infarction, and fibrinogen is involved in events thought to play a major role in thrombosis. Obesity itself increases the risk of coronary, and other vascular complications that frequently involve platelet activation and blood coagulation. The purpose of this study was to determine protein C, protein S activity and serum fibrinogen concentrations in subjects with obesity compared to non-obese subjects. Protein C, protein S activities and serum fibrinogen concentrations were studied in 30 obese and 30 non-obese subjects of the same age. Protein C, protein S activities and serum fibrinogen concentrations were measured by theuse of appropriate functional assays in both groups. Protein S activity was lower in the obese group (93.9±16.2%) as compared with the control group(102.7+19.8%). But there was no statistically significance between these two values(p=0.065). Protein C activity values were higher in the non-obese subjects (105.1 ±20.9%) than in the obese group (96.3+18.3%), but there was no statistical significance(p=0.088). Fibrinogen concentrations were found 370+51 and 247+43 mg/ml respectively in obese and non-obese groups. Fibrinogen values showed significant difference between two groups(p=0,001). The constellation of these findings suggest, obesity, itself may cause coronary ischemia, and other vascular complications by increased coagubility which occurs in the state of increased serum fibrinogen concentrations (and decreased protein C and protein S functions).

Protein C ve protein S, K vitamini bağımlı proteinlerdir. Doğal antikoagülan yolun merkezinde yer alırlar ve aktive protein C sayesinde fonksiyonlarını gerçekleştirirler. Bu proteinlerin etkili olduğu yol sadece koagülasyonu inhibe etmekle kalmaz, aynı zamanda fibrinolizisi de stimule eder. İnme ve myokard infarktüsünde ana mekanizmanın trombozis olduğu ve fibrinojenin trombozisi oluşturan olaylar zincirinde önemli rolünün varlığı, giderek daha fazla araştırmaya konu olmaktadır. Obezitenin trombosit aktivasyonu ve kan koagülasyonu ile ilişkili olarak koroner ve diğer vasküler olaylarda artışa neden olduğu ileri sürülmektedir. Bu çalışmanın amacı, obez ve obez olmayan kişilerdeki protein C, protein S ve serum fibrinojen konsantrasyonlarının belirlenmesi ve iki grup arasında fark olup olmadığının tespit edilmesidir. Otuz obez kişinin dahil edildiği çalışmada protein C, protein S ve serum fibrinojen konsantrasyonları belirlenerek obez olmayan 30 kişiden oluşmuş benzer yaştaki kontrol grubunun değerleriyle karşılaştırılmıştır. Obez grubun protein C aktivitesi, %96.3±18.3, kontrol grubunun protein C aktivitesi ise %105.1+20.9 olarak belirlendi. Obez grupta protein C aktivitesinde bir miktar düşüklük saptanmasına rağmen bu fark istatistiksel öneme sahip değildi(p-0.088). Protein S aktivitesi için ise obez ve obez olmayan gruplarda sırasıyla %93.9+16.2 ve %102.7+19.8 değerleri elde edildi. Ancak aradaki fark istatistiksel olarak anlamlı bulunmadı(p=0.065). Serum fibrinojen konsantrasyonları obez ve obez olmayan gruplarda sırasıyla 370+51 mg/ml ve 247±43 mg/ml olarak bulundu. Obez grupta serum fibrinojen konsantrasyonları belirgin biçimde yüksekti(p=0.001). Bu bulgularla; obezitenin artmış serum fibrinojen konsantrasyonu yoluyla, hiperkoagübiliteye neden olabileceği ve bu şekilde koroner iskemi ve diğer vasküler hastalıkları artırabileceği söylenebilir. Protein C ve protein S aktiviteleri yönünden daha büyük örneklem gruplu çalışmalara ihtiyaç vardır.
