Etiological evaluation of congenital cataract cases

Abstract

PURPOSE: To evaluate the etiology of cataract, children who have congenital cataract were assessed. METHOD: Sixty-three children who 0-12 mouth of age were included in the study The records of all the children were assessed for perinatal, prenatal and family history. After detailed ocular examination, a general and systemic, neuralgic and. developmental examination were made. Patients were investigated to elucidate the possible etiologic factors. Antibodies for rubella, toxoplasmosis, cytomegalo-virus and herpes-simplex virus were determined. Urine blood aminoacids were analyzed. RESULT: Etiological factors were determined as follows; 8/63 cases (12.6 %) were hereditary, 11/63 patients (17.5 %) had metabolic disorders (galactosemia 2, glycogen storage disease 3, mucopolysaccharidoses 2, I cell disease 1, aminpaaciduria 3), 23 / 63 patients (36.5: %) were affected with congenital infection (8 cytomegalo virus infection, 6 rubella, 6 toxoplasmosis, 3 herpes-simplex) and 2/63 had chromosomal abnormality (I case trisomv 21 and the next was 47 XXY). In 16/63 of the cases (25.4 %) etiology was unknown. CONCLUSION: These findings indicate that prenatal infections are still constituting the main cause of congenital ' cataract. In conclusion, besides the developmental, neurological and ocular examinations, laboratory analyses for congenital ..infection and inherited metabolic diseases should be performed for the assessment öf patients with congenital cataract:

Özet

AMAÇ: Konjenital katarakt nedeni ile izlenen hastalar katarakt etyolojisi yönünden araştırıldı. YÖNTEM: 0-12 ay arası toplam 63 hasta çalışmaya alındı. Hastalar ebeveynler arası akrabalık, aile hikayesi, mevcut anomali, annenin geçirdiği hastalıklar açısından sorgulandı. Sistemik muayeneleri yapıldı. Nörolojik, oftalmolojik ve gelişimleri yönünden değerlendirildi. Hastaların tümü kan-idrar aminoasitleri ve TORCH antikorları yönünden değerlendirildi. Gereken hastalarda genetik araştırma yapıldı. BULGULAR: Sekiz (%12.6) hastada ailesel geçiş, 11 (%17.5) hastada metabolik hastalık tespit edildi. Bu hastaların 2 si galaklozemi, 3'ii glikojen depo, 2 si mukopolisakkaridoz, 1 si I cell disease (mukolipidoz tip II), 3ünde ise aminoasidüri görüldü. TORCH infeksiyonu 23 (%36.5) hastada tespit edildi. Sitomegalovirus antikorları 8 hastada, rubella 5, toksoplazmosis 6, herpes simpleks virus antikorları 3 hastada positifti. İki hastada kromozom anomalisi görüldü. Biri trisomi 21, diğeri 47-XXY idi. Onaltı (%25.4) hastada ise etyoloji tespit edilemedi. SONUÇ: Konjenital infeksiyonların halen konjenital katarakt etyolojisinde çok önemli bir yer tuttuğu görüldü. Dikkatli bir nörolojik oftalmolojik muayene ve gelişim değerlendirmesinin katarakt etyolojisinin aydınlatılmasında çok önemli olduğu görüldü.
