Nonalcoholic and nondiabetic fatty liver: A clinical biochemical and histopathological study

Abstract

PURPOSE AND METHOD: Fatty liver is a histological entity associated with morbid obesity, maturity onset diabetes mellitus and alcoholism. The aim of this study is to investigate the clinical and laboratory findings of fatty liver and to determinate etiological factors that induce fatly liver in the patients who are nonalcoholic and nondiabetic. The 2030 biopsy specimens obtained between January 1988 and June 1992 were reviewed and 82 patients were selected to form the cohort of the study (46 female and 36 male; mean age, 43.8±12.5 years). RESULTS: Twenty-two patients (27%) had biochemical evidence of hyperlipidemia, 22 patients (27%) were obese and 11 (13%). were obese and hyperlipidemic. Fourty-nine patients (60%) had no obesity and hyperlipidemia. Other systemic disease, anemia, malnurition and drug toxicity were found as cause of fatty liver in 42 (51%) patients, and in 7 patients, (9%) no etiologic factor was found. Histological findings were fatty liver in 39 (47%) patients; fatty hepatitis in 28 (34%) patients, fatty fibrosis in 12 (15%) patients, and fatty cirrhosis in 3(4%) patients. Two patients with cirrhosis were hepatitis C positive. CONCLUSION: This study suggests that obesity and hyperlipidemia are the most important causes of fatty liver in nonalcoholic and nondiabetic patients. Obese and hyperlipidemic patients should be taken on a diet programme and followed up.

AMAÇ VE YÖNTEM: Hepatosteatoz, morbid obesite, erişkin tip diabetes mellitus ve alkolizm ile birlikte sık bulunan histolojik bulgudur. Bu çalışma, nonalkolik ve nondiabetik hastalarda hepatosteatozun klinik ve laboratuvar bulgularını araştırmak ve hepatosteatoza yol açan etiyolojik faktörleri belirlemek için düzenlendi. Ocak 1988 ve Haziran 1992 yılları arasında yapılan 2030 biyopsi taranarak, aradığımız özelliklerde 82 hasta çalışma kapsamına alındı (46 kadın, 36 erkek; yaş ortalaması 43.8±12.5 yıl). BULGULAR: Yirmi iki hasta (%27) hiperlipidemik, 22 hasta (%27) obez, 11 hasta (%13) hem obez hem de hiperlipidemik idi. Kırk dokuz hastada (%60) obesite veya hiperlipidemi yoktu. Bu hastaların 42' sinde (%51). etiyolojide değişik sistemik hastalıklar; anemi, malnütrüsyon, ilaç toksisitesi saptandı. Yedi hastada steatoz etiyolojisi saptanamadı ve kriptojenik olarak kabul edildi. Karaciğerdeki lezyonların histolojik değerlendirmesinde, steatoz 39 vakada (%47), steatohepatit 28 vakada (%34), steatofibroz 12 vakada (%15), steatosiroz 3 vakada (%4) tespit edildi. Üç steatosiroz vakasının ikisinin etiyolojisinde hepatit C virüs enfeksiyonu vardı. SONUÇ: Obesite ve hiperlipidemi, alkol ve diabet dışı hepatosteatozun en önemli nedenidir. Obez ve hiperlipidemik hastalar uygun diyet programına alınmalı ve izlenmelidir.
