Necrotizing soft tissue infections

Abstract

OBJECTIVE:To evaluate the causes of necrotizing soft-tissue infections (NSTI) and to determine the outcomes of patients with NSTIs. METHOD:Ten patients admitted to Ankara Hospital 1. General Surgery Department with the diagnosis of necrotising soft, tissue infections between May 1995 - January 1999were evaluated retrospectvely. RESULTS: Four of the patients were male and six of them were females, whereas the mean age of the group was 49.8+11.8. The etiologic factor was diabetes mellitus in 5 patients (%50), trauma in 2 patients (%20) and subcutaneus prganophosphate injection 1 in patient (%10). The necrotic sites were mainly on the perineum, lower limbs and vulva. Their minimum necrotic area size was 180 cm 2 and the maximum necrotic area was 720 cm 2 (medn 216+165.05 cm 2 ). The only patient who died was the one with the largest necrotic area (mortality 10 %). Low. hemoglobin levels (70 %), leucocytosis (60 %) and hyperglisemia were the most frequently encountered pathologic: laboratory values. On 40 percent of the wound cultures there were polymicrobiaigrowths, and appropriate, antibiotic therapies were planned. All of the patients were observed with aggresive debridmans, daily wound'cares and supportive, therapy. The defects were closed primarily (40 %), by auto grafts (30 %) and by flaprotations (20 %). On one patient an end colostomy was performed to avoid wound contamination. CONCLUSIONS: Our mortality rate maches with the rates of larger series in the literature. It should be kept in mind that the rationale in the treatment of necrotizing soft tissue infections is early and effective wound debridement.

AMAÇ: Kliniğimize yatırılan Nekrotizan Yumuşak Doku Enfeksiyonlu hastaların değerlendirilmesi, prognozu etkileyen faktörler ve tedavi prensiplerini irdelemek, YÖNTEM: Mayısl995 -Ocak 1999 yılları arasında S.B. Ankara Eğitim ve Araştırma Hastanesi 1. Genel Cerrahi Kliniğine Nekrotizan Yumuşak Doku Enfeksiyonu nedeniyle yatırılan 10 hasta yaş, cins, etyoloji, tedavi ve prognozları açısından retrospektif olarak incelenmiştir. BULGULAR: Altısı kadın, 4'ü erkek olan hastaların yaş ortalaması 49.8+11.8 olarak bulunmuştur. Genişlik olarak en az 180 cm 2, en çok 720 cm2 (ortalama 216+165.0.5 cm2) alanın tutulduğu ve etyolojilerinde %50 Diabetes Mellitus, %20 travma, %10 Sııbkutan Organofosfat enjeksiyonu olduğu saptanmıştır. Nekroz alanlarının sıklıkla perine, alt ekstremiteler ve vulvada yerleştiği ve en geniş tutulum gösteren hastanın olgular içinde tek kaybedilen hasta olduğu saptanmıştır (Mortalite %10). En sık patolojik laboratuvar tetkikinin hemoglobin düzeylerinde düşüklük (%70) olduğu ve lökositoz (%60) ile hipergliseminin (%50) bunu izlediği görülmüştür. Yapılan kültürlerde %40 polimikrobiyal üreme olmuş ve uygun antibiyotik tedavileri verilmiştir. Hastaların hepsine geniş cerrahi debridman zaman kaybetmeden uygulanmış, günlük pansumanlar ve destek tedavileri ile hastalar izlenmiştir. Debridmanı takiben hastaların %40 'ma sekonder kapatma, %30 'una otogreft, %20 'sine flep rotasyonu uygulanmıştır. Bir olguda kontaminasyonu engellemek amacıyla geçici kolostomi uygulanmıştır. SONUÇ: Çalışmamız büyük serilerle karşılaştırıldığında, mortalite oranı literatürle uyumlu olarak bulunmuştur. Bu hastalığın tedavisinde zaman kaybından kaçınmanın ve yeterli cerrahi debridmanın en önemli tedavi prensipleri olduğu unutulmamalıdır.
