Termination of second trimester pregnancy with oral and vaginal misoprostol combination

Abstract
OBJECTIVE: To observe the efficacy of oral and vaginal combination of misoprostol (prostoglandin E2 analogue) in termination of 2nd trimester pregnancies. MATERIAL AND METHOD: 26 patients with a diagnosis of intrauterine exitus, fetal anomaly and history of teratogenic drug we were enrolled into this study. 23 patients were diagnosed intrauterine exitus, 2 had fetal anomalies (hydrocephahis) and I had an history of teratogenic drug use. All patients received an overnight oral dose of misoprostol (200 meg) at 03.00 AM and at 8 AM they received a dose of 200 meg misoprostol vaginallv. They all received oral 200 mcg misoprostol in 4 hours intervals until the abortion of the fetus. RESULTS: After the application of first oral dose succesfull termination was detected in 2 patients in 8 hours. 17 patients between 8-12 hours and 2 patients between 24-48 hours. The mean first oral dose to abortion time was 9.5 hrs in this group of patients. CONCLUSION: Application of vaginal dose after the first oral dose was found to be more efficacious and easier. Nearly all patients were terminated in day time. Combination of oral and vaginal misoprostol applications were easy, effective, cheap and without any side effects for the termination of 2nd trimester pregnancies.

AMAÇ: Son zamanlarda 2. trimesir gebeliklerin sonlandırılmasında sıklıkla kullanılan Prostoglandin E2 analogu Misoprostol'ü oral ve vaginal kombine kullanarak etkinliğini araştırmak. MATERYAL VE METOD: Çalışma intrauterin exitus, fötal anomali, bilmeyerek teratojenik ilaç kullanımı tanısıyla hastaneye yatırılan 26 hasta üzerinde yapıldı. 23 hastada intrauterin exitus, 2 hastada fötal anomali (hidrosefali), 1 hastada teratojenik ilaç kullanımı öyküsü vardı. Hastalara abort zamanını tercihan mesai saatleri içerisinde tutabilmek amacıyla saat 03.00'le 1 adet misoprostol tablet oral yolla verildi. Sabah saat 08.00'de 200 mcg Misoprostol tablet vaginal yerleştirildi, abort sağlanana kadar 4 saat ara ile 1 tablet oral verilmeyi; devam edildi. BULGULAR: İlk olarak ilaç uygulamasını takiben 8 saat içerisinde 2 hasta, 8-12 saat içerisinde 17 hasla, 12-24 saat içerisinde 5 hasta. 24-48 saat içerisinde 2 hastada abort sağlandı. Ortalama abort suresi 9.5 saat idi.SONUÇ: Çalışmamızda oral misoprostol tablet verilmesini takiben olgunlaşmış servikse ilaç uygulaması daha kolay olmuştur. Vakaların çoğunda mesai saatleri içerisinde abort sağlanmıştır. Misoprostol'ün oral ve vaginal kombine kullanımı, uygulaması kolay etkin, ucu: ve yan etkisi az olan bir yöntemdir.
