Importance of determining serum haptoglobulin levels in hypertensive pregnant women

Abstract
OBJECT: To evaluate the importance of determining haptoglobulin levels as a hemolysis marker and prognostic factor in hypertensive pregnant women. MATERIALS AND METHODS: This prospective study included 41 hypertensive pregnant women and 21 healthy controls, most of whom were in the third trimester. On the basis of case histories, physical and laboratory findings patients were divided into five groups as chronic hypertensive, mild or severe preeclamptic, eclamptic, and those with HELLP syndrome. In all pregnant women total bilirubin, LDH, SGOT, SGPT, platelets, and haptoglobulin levels were measured for the differential diagnosis of HELLP syndrome and for the evaluation of hemolysis. Moreover for each patient in the study group we evaluated maternal age, pregnancy week and number of pregnancy, blood pressure, proteinurea, edema, total blood count, total blood biochemistry, type of delivery, and the APGAR score and the weight of the newborn. The results were analysed using Student-t test and x2 test. RESULTS: The complicating effect of HELLP syndrome in preeclampsia and eclampsia increases materno-fetal mortality and morbidity. Compared to the other groups, in patients with HELLP syndrome, we found significantly decreased haptoglobulin and platelet values, and elevated LDH and bilirubin levels. In severe preedamptic group, there were significant decreases in haptoglobulin levels compared to the control group. We also found correlation between decreased haptoglobulin and decreased newborn weight. In pregnant women with a history of convulsions and/or coma indicators for hemolysis were significantly abnormal and perinatal mortality high. CONCLUSION: In view of our findings, HELLP syndrome should be considered in the presence of nonspecific complaints of pregnant women. Haptoglobulin analysis, which proved valuable in the early diagnosis and prognosis of hemolysis, should be included in routine laboratory tests.

AMAÇ: Gebeliğin hipertansif hastalığında haptoglobin tayininin hemoliz markeri ve prognoslik faktör olarak önemi araştırıldı. MATERYAL VE METOD: Bu çalışma çoğunluğu 3. trimesterde 41 hipertansif ve 21 sağlıklı kontrol grubu gebede prospektif olarak yapıldı. Hastalar anamnez fizik ve laboratuvar bulgularına göre kronik hipertansif, hafif ve ağır preeklamptik, eklamptik ve HELLP sendromlu olarak 5 gruba ayrıldı. Tüm gebelerde HELLP sendromu ayırıcı tanısı ve hemolizin değerlendirilmesi için total bilüribin, LDH, SGOT,SGPT, trombosit ve haptoglobin ölçümleri yapıldı. Ayrıca tüm çalışma grubunda maternal yaş, gebelik haftası ve sayısı, kan basıncı, proteinüri, ödem, tam kan sayımı, tam kan biyokimyası, doğum şekli, yenidoğan APGAR ve ağırlığı değerlendirildi. Sonuçlar Student-t ve X2 testleriyle değerlendirildi. BULGULAR: Preeklampsi veya eklampsinin HELLP sendromuyla komplike olması materno-fetal mortalite ve morbiditeyi artırmaktadır. Araştırmamızda haptoglobin HELLP sendromunda diğer gruptakilere göre anlamlı derecede azalmış bulundu. Aynı zamanda azalmış haptoglobinle düşük yenidoğan ağırlığı arasındaki ilişki anlamlıydı. Konvülziyon / koma kliniği olan gebelerde hemoliz göstergeleri anormal ve perinatal mortalite anlamlı olarak yüksekti. SONUÇ: Bu bulgularla, gebelerde nonspesifik yakınmaların varlığında HELLP sendromu akla gelmeli, hemolizin erken tanısı ve prognozun kestirebilmesinde yararlı bir tanı aracı olan haptoglobin analizi rutin laboratuar testleri arasında yer almalıdır.
