The relation between the stage of chronic liver diseases caused by viruses and iron metabolism

Abstract
AIM: The abnormalities of iron metabolism in the chronic liver disease caused by viruses and the relation between response to interferon treatment and the iron content in the liver have been reported. The aim of our study was to determine the relation between the stage of chronic liver disease caused by viruses and iron metabolism. MATERIALS AND METHODS: Twenty-eight patients with a mean age of 38.6 years (range, 27-58) who had diagnosis of chronic liver disease caused by viruses including 10 decompensated cirrhosis, 10 compensated cirrhosis, 9 chronic active hepatitis and 11 healthy subjects with a mean age of 39 (range, 25-52) were studied. In all cases, serum iron levels (SI), iron binding capacity of serum (IBCS), transferrine saturation (TS). ferritin, the changes in the levels of serum iron after oral iron administration, and the changes in the levels of iron excreted in the urine by Desferrioxamine B as well as the presence of iron accumulation in the liver tissue of patients were determined. RESULTS: The levels of SI, TS and ferritin were significantly higher in the decompensated cirrhosis group compared to the others three groups (p<0.05, p<0.005, and p<0.05, respectively). In the. decompensated cirrhosis group, the changes in the levels of serum iron compared to baseline levels at the first, second, third, and fourth hours after oral iron administration were significantly lower than others three groups (p<0.05, p<0.005, p< 0.005, and p< 0.005, respectively}. The levels of iron in the liver tissue and the levels of iron excreted by Desferrioxamine B were significantly higher in the decompensated cirrhosis group compared to other three groups (p<0.01 and p< 0.05. respectively). There were no significant difference between compensated cirrhosis, chronic active hepatitis and control group according to any parameters. CONCLUSION: In the advanced stages of liver disease, decreased iron absorption in the intestine, impaired iron metabolism, and pathologic iron accumulation in the liver, which can be mobilized by Desferrioxamine B, were determined.

AMAÇ: Virüslere bağlı kronik karaciğer hastalığında demir metabolizmasının bozulduğu ve karaciğerdeki demir miktarıyla interferon tedavisine cevap arasında ilişki olduğu bildirilmiştir. Çalışmamızın amacı virüslere bağlı kronik karaciğer hastalığının evresi ile demir metabolizması arasındaki ilişkiyi belirlemekti. GEREÇ VE YÖNTEM: Çalışmaya virüslere bağlı karaciğer hastalığı tanısı konulan, ortalama yaşları 38.6 (27-58) yıl olan 10 dekompanse siroz, 10 kompanse siroz, 9 kronik aktif hepatit olmak üzere toplam 29 hasta ve ortalama yaşları 39 (25-52) yıl olan 11 normal sağlıklı birey alındı. Tüm vakalarda serum demiri (SD), serum demir bağlama kapasitesi (SDBK), transferrin saturasyonu (TS), ferritin, oral demir alınımı sonrası serum demir düzeyindeki değişiklik ve desferrooxamine B ile idrarla atılan demir miktarı ölçüldü. Tüm hastalarda karaciğer biyopsisinde demir varlığı histolojik yöntemle araştırıldı. BULGULAR: Dekompanse siroz grubunda SD, TS ve ferritin düzeyleri diğer üç gruba göre anlamlı olarak yüksek saptandı (sıra ile p<0.01, p<0.005 ve p<0.05). Oral demir alınımı sonrası birinci, ikinci, üçüncü ve dördüncü saatlerde serum demir seviyesinde bazal seviyeye göre meydana gelen değişiklik dekompanse siroz gurubunda diğer üç gruba göre anlamlı olarak düşük (sıra ile p<0.05, p<0.005, p<0.005 ve p<0.005) bulundu. Karaciğer biyopsisinde demir varlığı ve desferrooxamine B sonrası idrarla atılan demir miktarı dekompanse siroz grubunda diğer gruplara göre anlamlı olarak yüksek bulundu (sıra ile p<0.01 ve p<0.05). Kompanse siroz, kronik aktif hepatit ve kontrol grubu arasında tüm parametreler açısında anlamlı fark saptanmadı. SONUÇ: Kronik karaciğer hastalığının ileri evrelerinde barsaklardan demir emiliminin azaldığı, demir metabolizmasının bozulduğu, karaciğerde desferoxamine B ile mobilize edilebilen patolojik demir birikiminin olduğu saptandı.
