Abstract

AIM: Searching of the effect of anesthesia and surgery on blood parameters. MATERIAL AND METHODS: Peripheral venous blood was taken from 43 female breast carsinoma patients before the radical modified radical mastectomy, immadiately afterwards the operation when patients were in the recovery room and after 7 to 10 days postoperatively.These specimens were taken preoperative, peroperative and postoperative .Leucocyte and leucocyte formula (neutrophil, eosinophil, basophil, lymphocyte and monocyte) was studied.No patient was taken blood transfusion or chemotherapy during the study. Paired Samples T test and pearson test was used for the statistical analysis. RESULTS : Peroperatively neutrophyl, eosynophyl and monocyte absolute numbers has been increased and lymphocyte numbers has been decreased (p< 0.02).Exept of the lymphocyte numbers; the others turned to normal values at the 10th postoperative day. CONCLUSION:We concluded that the lymphocyte numbers depressed after the modified radical mastectomy which suggests an immunodepression in the study group. Lymphocytes consists of B and T cells which is called specific immunity and natural killer cells which are parts of non-spesific immunity( 1,2,3) and both of them acts against cancer disease(l,2).Deacrease of lymphocyte number could be estimated as immunodepression in the study group. Further studies are needed to show which subgroups of the lymphocyte are depressed by the anesthesic and surgical strees and its effect on the prognosis of breast cancer must be evaluated.

AMAÇ: Ankara Onkoloji Eğitim veAraştırma Hastanesinde meme kanseri tanısıyla modifiye radikal mastektomi uygulanan, neoadjuvan kemoterapi almamış, preoperatif, peroperatif ve postoperatif kan transfüzyonu yapılmamış 43 kadın hasta anestezi ve cerrahi stresin kan parametleri üzerine olan etkilerinin araştırılması amacıyla değerlendirildi. GEREÇ VE YÖNTEM: Hastaların hepsinden ameliyattan bir gün önce, ameliyat bitimini takiben hasta yoğun bakıma alınır alınmaz ve postoperatif 7. ve 10. günlerde intravenöz kan alınarak lokosit ve lokosit formülleri (nötrofıl, eozinofıl, bazofıl, lenfosit ve monosit) çalışıldı. Preoperatif, peroperatif ve postoperatif lokosit sayıları ve lokosit formülleri eşzamanda iki ölçüm arasındaki farkın önemlilik testi ile (bağımlı örneklerde student T testi) [Paired Samples T test] karşılaştırıldı (p< 0.05) anlamlı kabul edildi.Tüm beyaz küre alt gruplarının aralarında pozitif veya negatif yönde ilişki olup olmadığı Pearson korelasyon testi ile karşılaştırıldı. BULGULAR: Peroperatif nötrofıl, monosit ve eozinofıl sayısında artış olduğu saptandı.Bu sonuç istatistiksel olarak anlamhydı(p<0.05).Lenfosit sayısının ise peroperatif olarak düştüğü bulundu.Bu sonuçta istatistiksel açıdan anlamlıydı(p<0.02).Nötrofıl, monosit ve eozinofıl sayısı postoperatif 10. günde normale dönerken, lenfositlerin preoperatif değerlerineulaşmadıkları görüldü.Bu bulguda istatistiksel olarak anlamlıydı(p<0.02). SONUÇ: Anestezi ve cerrahi stresin kan parametrelerinden lenfosit sayısını azlttığı ve bununda immün sistemin baskılanması anlamına gelebileceği sonucuna varıldı. Lenfositler spesifik immünitede rol alan B ve T hücrelerini, nonspesifik immünitede rol alan natural killer hücrelerini kapsadığından (1,2,3) ve bu iki sistemde kansere karşı immünitede rol aldıklarından (1,2), anestezi ve cerrahi stresin lenfositlerin hangi subgrup veya subgruplarını baskıladığı ve bunun meme kanseri prognozu üzerine etkisinin ileri çalışmalarla araştırılması gerektiği sonucuna varıldı.
