The investigation of changes in humoral and cellular immunity in patients with active brucellosis

Abstract

PURPOSE: In this study, the changes in humoral and cellular immunity in 21 patients (12 men and 9 females) with active brucellosis were examined. METHODS: 10 healty volunteers (6 men and 4 females) were included to the study and served as a control group. The diagnosis of the active brucellosis was made by the Wright agglutination and 2-mercaptoethanol tests. Serum IgG, IgM, IgA, C3c and C4 fractions of the complement system were measured by nephelometry for nonspesiflc humoral immunity. Lymphocyte subpopulations and proportions of CD4 and CDS were examined by flow cytometry. Phagocytic activity of monocytes and granulocytes were measured by flow cytometry. RESULTS: The differences between the patients and control groups for the serum immunoglobulin, C3c and C4 levels, and phagositic activity of monocytes and granulocytes were found to be statistically insignificant (p>0.05). There were also no significant differences between the proportions of CD19, CD4, CDS, natural killer cells and CD4/CD8 in both groups (p>0.05). However, the proportions ofCD3 and activated T lymphocytes in patients were found to be significantly elevated when compared to healty volunteers (p<0.01 and p<0.001). CONCLUSION: In conclusion, we found a significant increase in the activation of T lymphocytes in active brucellosis. Therefore, these results suggest that the immune response against the Brucella bacteries which have the ability of intracellular growth in humans is developed under the control of T lymphocytes.

AMAÇ: Aktif brusellozlu hastalarda meydana gelebilecek olası immun yanıt değişikliklerinin belirlenmesi amaçlanmıştır. YÖNTEM: Bu çalışmada aktif brusellozu olan 12'si erkek, 9'u, kadın 21 hastadaki humoral ve hücresel immün yanıt değişiklikleri incelendi. Kontrol grubu olarak 6 erkek ve 4 kadından oluşan 10 sağlıklı gönüllü çalışmaya alındı. Hastaların tanısı klinik bulgular, Wright agglütinasyon ve 2-merkaptoetanol testi ile kondu. Hasta grubu (H.G.) ve kontrol grubunun (K.G.): serum IgG, IgM, IgA, kompleman C3c, C4 düzeyleri, lenfosit subpopülasyonları, CD4/CD8 oranları, monosit ve granülosit fagositik aktiviteleri ölçüldü. BULGULAR: H.G. ile K.G.'nün serum immünglobulinleri, C3c ve C4 düzeyleri, granülosit ve monosit fagositik aktiviteleri arasında istatistiksel olarak anlamlı fark saptanamadı (p>0.05). H.G. ve K.G.'nün lenfosit subpopülasyonlarından CD19, CD4, CDS, Natural Killer hücre oranları ve CD4/CD8 oranları arasında anlamlı fark saptanamadı (p>0.05). Ancak CD3 ve Aktive T lenfosit oranları H.G'da K.G'na göre istatistiksel olarak, anlamlı ölçüde yüksek bulundu (p<0.01 ve p<0.001). Bu çalışmada aktif brusellozlu hastalarda T lenfosit aktivasyonunda belirgin artış saptanmıştır. SONUÇ: Bu artış insanlarda Brucella bakterilerine karşı immün yanıtın, T lenfositlerinin kontrolünde geliştiğini gösterebilir.
