Lipid-lipoprotein values in pregnancy trimesters at the healthy pregnants and three months of the lactation periods

Abstract

OBJECTIVE: Blood lipid and lipoprotein levels have been found to change in every trimester of a uncomplicated pregnancy. This study has been performed to find out whether blood lipid&lipoprotein levels differ in alternating 3 months during lactation. MATERIAL AND METHODS: One hundred thirty three healthy pregnant patients admitted to Gynecology & Obstetrics Department of Kırıkkale University Hospital between June 1998 and December 1999 were included in the study. Serum total chollesterol, triglyceride, high density lipoprotein-chollesterol, low density lipoprotein-chollesterol, very low density lipoprotein-chollesterol levels and total chollesterol/high density lipoprotein-chollesterol, low density lipoprotein-chollesterol/high density lipoprotein-chollesterol ratios have been determined in each trimester of pregnancy and every three months for 4 times after the labor in lactation period. RESULTS: We found prominent statistically significant increase in serum levels of total chollesterol, very low density lipoprotein-chollesterol, in 2-3nd trimester of pregnancy (p=0.000, p<0.05). Serum levels of trigliceride was at the high ranges from the first trimester to the whole of the pregnancy in spite of at lactation periods was receded to significant normals values (p=0.000, p<0.05). Increase in serum levels of low density lipoprotein-chollesterol was also statistically significant but less prominent (p=0.039, p<0.05). Serum levels of high density lipoprotein-chollesterol has not been seem insignificant change at this time. The ratio of total chollesterol/high density lipoprotein-chollesterol and low density lipoprotein-chollesterol/high density lipoprotein-chollesterol seemed to change during progression of pregnancy but was statistically insignificant (p=0.479, P>0.05). CONCLUSION: Hyperlipidemic and hypercholesterolemic during pregnancy probably has a important role in providing maternal haemostasis, placental steroid synthesis and feeding of the fetus. The increase in lipid&lipoprotein levels is maintained during lactation also.

AMAÇ: Bu çalışma, normal gebeliğin ilerleyişi sürecinde; gebelik trimestirlerine göre kan lipid ve lipoprotein düzeylerinde görülen değişikliklerin, üçer aylık dört laktasyon periyodunda da devam edip etmediğini araştırmak amacıyla yapıldı. GEREÇ VE YÖNTEM: Kırıkkale Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesi Kadın Hastalıkları Doğum AD polikliniğine Haziran 1998 - Aralık 1999 tarihlerinde başvuran 133 sağlıklı gebenin gebelik trimestirleri ve üçer aylık laktasyon periyodlarında serum total kolesterol, trigliserid, çok düşük yoğunluklu lipoprotein-kolesterol, düşük yoğunluklu lipoprotein-kolesterol, yüksek yoğunluklu lipoprotein-kolesterol, total kolesterol/ yüksek yoğunluklu lipoprotein-kolesterol ve düşük yoğunluklu lipoprotein-kolesterol/ yüksek yoğunluklu lipoprotein-kolesterol oranlarındaki değişiklikler değerlendirildi. BULGULAR: Gebeliğin 2-3'üncü trimestirinde serum total kolesterol ve çok düşük yoğunluklu lipoprotein-kolesterol serum düzeylerinin diğer gebelik trimestirlerine göre çok belirgin şekilde istatistiksel anlamlı (p=0.000, p 0.05) saptandı. Gebelik ve laktasyon süreçlerinde; serum total kolesterol/ yüksek yoğunluklu lipoprotein-kolesterol ve düşük yoğunluklu lipoprotein-kolesterol/ yüksek yoğunluklu lipoprotein-kolesterol oranlarında anlamlı olmayan değişiklikler saptandı (p>0.05). SONUÇ: Bu çalışmada, gebelik dönemlerinde görülen hiperlipidemi ve hiperkolesterolemi'nin maternal hemostaz'a yardımcı faktör, plasental steroid sentezi ve fetal enerji için kaynak oluşturmaya yönelik olduğu, laktasyon dönemlerinde ise laktasyon'un ikinci-üçüncü ayındaki lipid ve lipoprotein düzeylerinin önemli bir değişiklik göstermeksizin devam ettiği belirlendi.
