Is uric acid measurements useful in hypertensive pathologies of pregnancy?

Abstract
OBJECTIVE: The aim of this study is to investigate the usefulness of serum uric acid levels in hypertensive states of pregnancy. MATERIAL AND METHOD: Serum uric acid levels were measured in 30 normotensive and 140 (transient hypertension n=22, preeclampsia n= 73, chronic hypertension n=19, superimposed preeclampsia n=26) hypertensive pregnants who have completed 36th weeks of gestation and predictive values of the test were calculated for preeclampsia and superimposed preeclampsia. The likelihood ratios were also calculated for positive and negative test results for the level of uric acid at the level of 5.5 mg/dl. RESULTS: The uric acid levels were found to be 3.9±0.9 mg/dl, while it was 5.4±l.8 mg/dl in transient hipertension, 6.51-1.2 mg/dl in preeclampsia and 5. 7±1.3 mg/dl in superimposed preeclampsia. When compared with normotensive group the difference was statistically higher (p<0.05). On the other hand, the difference of uric acid levels between normotensive and chronic hypertensive pregnants was insignificant (p>0.05). When 5.5mg/dl was used us a cut-off value, the sensitivity and specifisity of the test in discriminating transient hypertension and preeclampsia were 70% and 54%, respectively. With the same uric acid level discriminating chronic hypertension and superimposed preeclampsia by using this test was 58% sensitive and 82% specific. The likelihood ratio of having preeclampsia at the basis of transient hypertension was found to be 1.52, while it was 3.2 having superimposed preeclampsia at the basis of chronic hypertension. CONCLUSION: Serial uric acid measurements are useful and necessary in chronic hypertensive pregnants in predicting superimposed preeclampsia. However, in transient hypertensive pregnants this test is not useful and does not help to clinicians in monitoring patients.

AMAÇ: Gebeliğin hipertansiyon ile seyrettiği durumlarda serum ürik asit ölçümlerinin yararını araştırmak. GEREÇ VE YÖNTEM: Gebeliğin 36. haftasını tamamlamış 30 normotansif ve 140 hipertansif vakada (transient hipertansiyon n=22, preeklampsi n=73, kronik hipertansiyon n= 19. superimpoze preeklampsi n=26) senim ürik asil seviyeleri ölçülerek preeklampsi ve superimpose preeklampsinin ayrımındaki kestirim değerleri saptanmıştır. Eşik değer 5.5mg/dl deki ürik asit seviyesine göre testin pozitif ve negatif test sonuçlarına göre olasılık oranları hesaplanmıştır. BULGULAR: Ürik asit değerleri 30 term gebede 3.9±0.9 mg/dl, transient hipertansif gebelerde 5.4±1.8 mg dl, preeklamptik gebelerde 6.5±12 mg/dl ve superimpoze preeklampsi vakalarında 5.7±1.3 mg/dl olarak saptanmış ve bu değerlerin tümünün kontrol grubuna göre istatistiki olarak yüksek olduğu görülmüştür (p 0.05). Çalışmada transient hipertansiyonu olan hastalarla preeklampsi gelişen vakaların ayrımında eşik değer olarak kabul edilen 5.5mg/dl ürik asit seviyesinin sensitivitesi %70, spesifisitesi ise %54 olarak hesaplanmıştır. Öte yandan kronik hipertansiyon ile bu zeminde gelişen superimpoze preeklampsinin ayrımında aynı eşik değer kullanıldığında test %58 sensitif, %82 spesifiktir. Buna göre transient hipertansiyon zemininde preeklampsi gelişiminin olasılık oranı hesaplandığında, bu eşik değer için 1.52 olarak bulunurken, kronik hipertansiyon vakalarında gelişebilecek superimpoze preeklampsinin olasılık oranı. 3.2 olarak hesaplanmıştır. SONUÇ: Kronik hipertansiyonu olan gebelerde seri ürik asil tayinleri yapmak faydalı ve bazen de yaklaşan superimpoze preeklampsiyi önceden kestirmek açısından değerli bir yaklaşımken, transient hipertansiyon vakalarında ürik asit değerlerinin tayini klinik açıdan yararlı değildir ve hasta monitorizasyonunda klinisyene yardımcı olamaz.
