Effect of postoperative human growth hormone treatment on wound healing in experimental colonic anastomosis

Abstract

OBJECTIVE: The aim of the study is to investigate the effect of biosynthetic human growth hormone (b-hGH) on anastomotic wound healing. MATERIAL AND METHOD: Rats were randomised into four groups, ten rats for each group. Hormone groups were treated with subcutane b-hGH 3 IU/kg/day in the 3 rd and 7 th postoperative days and control groups received sc normal saline administration. At the end of the experiment anastomotic bursting pressures were measured. Hydroxiproline levels in the tissue samples taken from the anastomotic lines were also measured and histopathologic interpretations were done. RESULTS: Bursting pressure measurements in the 3 rd postoperative day revealed a statistically significant difference between control and hormone treated groups (p<0.01). However, there was no difference between the groups regarding hydroxyproline levels. But when we compare the hydroxyproline levels in b-hGH treated groups hydroxyproline levels in the 7 th postoperative day was found to be significantly higher then the 3 rd day levels. Histopathologic examinations revealed a statistically significant difference (p<0.01) between 3 rd and 7 th days regarding mononuclear cells, fibrin, fıbroblast, collagen and neovascularisation. CONCLUSION: Postoperative b-hGH treatment seems to have a positive effect on left colonic anastomosis in the rats.

Özet

AMAÇ: Çalışmanın amacı sol kolon rezeksiyonu ve uç uca anastomoz yapılan ratlarda postoperatif verilen biyosentetik insan büyüme hormonun 'un (b-hGH) anastomoz yara iyileşmesi üzerine etkisini araştırmaktır. GEREÇ VE YÖNTEM: Çalışmamızda ratlar, her biri 10 denekten oluşan 4 gruba ayrıldı. Hormon gruplarına postoperatif 3 gün ve 7 gün 3 IU/kg/gün cilt altı, b-hGH, kontrol gruplarına ise aynı günlerde cilt altı salin verildi. Çalışma sonunda anastomoz patlama basınçları ölçüldü. Anastomoz hattından doku örnekleri alınarak, doku hidroksiprolin düzeyi ölçümleri yanısıra histopatolojik incelemeler yapıldı. BULGULAR: Patlama basıncı ölçümlerinde, postoperatif 3. günde b-hGH verilen grup ile kontrol grubu arasında istatistiksel olarak anlamlı fark mevcuttu (p<0.0l). Hidroksiprolin düzeyi ölçümleri açısından ise gruplar arasında fark yoktu. Ancak b-hGH verilen grupların kendi içinde yapılan karşılaştırmada 7. gün hidroksiprolin değerleri, 3. gün değerlerinden anlamlı yüksek bulundu. Histopatolojik incelemede ise 3. ve 7. gün sonunda b-hGh verilen grup ile kontrol grubu arasında mononükleer hücreler, fibrin, fibroblast, kollajen ve neovaskülarizasyon açısından anlamlı fark mevcuttu(p<0.01). SONUÇ: Sonuç olarak rotlarda sol kolonda yapılan anastomozda, postoperatif verilen b-hGH'un anastomoz iyileşmesi üzerine olumlu etkisi olduğu düşünüldü.
