Abstract

55-year-old patient was admitted with symptoms of urinary incontinence. After the preoperative evaluation, TVT procedure was performed under the epidural anesthesia. Technically minimal vaginal dissection was performed as originally described. After the application of the mesh, lateral urethral perforation was detected by cystoscopy. The neddle was retracted and the vaginal dissection was enlarged. Urethral perforation was due to periurethral adhesions that occurred after previous operations causing displacement of the urethra and this could be prevented by adequate urethrolysis.

Özet

55 yaşındaki üriner ınkontinans yakınmasıyla başvurdu. Preoperatif değerlendirmeden sonra, epidural anestezi altında TVT operasyonu uygulandı. Teknik olarak operasyonun orijinal tarifinde olduğu gibi minimal vaginal disseksiyon yapıldı. TVT uygulandıktan sonra yapılan sistoskopide uretranın lateral duvarında perforasyon gözlendi. İğne geri çekilerek, vaginal disseksiyon genişletildi. Üretral perforasyonun daha önce geçirilmiş operasyona bağlı olarak üretra çevresinde oluşan yapışıklıklar dolayısıyla uretranın yer değiştirmesine bağlı olduğu ve yeterli uretrolizisle önlenebileceği saptandı.
