First trimester ultrasonography findings in patients with abortus imminence

Abstract

Objective:To emphasize the importance of obstetric ultrasonography wich is performed for the patients of first trimester abortus imminens. Materıal and Methods:229 patients who were applied with abortus imminens findings to SSK Ege Maternity Hospital gynecology policlinic between 01.09.2000 and 30.11.2000 were evalueted.Of the 69 patients were removed from the study because of unsufficient follow up and because of not being in the first trimester.Obstetric ultrasonography is performed to the patients who applied for the vaginal bleeding in the first trimester by Kranzbuchler sonoscop 3 G 3.5 mHz.Twice. Results:Mean age was 28 (17-43).Mean gravity was 2, mean parity was 1.Vaginal bleeding continuing for 1 to 9 days was the application reason to hospital for all of the patients.Of the 72 patients (%45) lower abdominal pain and abdominal cramp were existed.Of the 90 (%56.2) patients were normal pregnancy, of the 36 (%22.5) patients were incomplet abortus, of the 19 (%11.8) patients were missed abortus, of the 14 (%8,75) patients were anembrionic sac and one of the patients (%0,75) was ectopic pregnancy.Gestational age by ultrasonography was 7 weeks and 6 days. Conclusıon:Obstetric ultrasonography research in the first trimester has a great importance for the health, morbidity and mortality of pregnants.

Amaç:Birinci trimester abortus imminensli olgularda yapılan obstetrik ultrasonografinin komplike gebeliklerin ayrıcı tanısındaki önemini vurgulamak. Gereç ve Yöntem:SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi jinekoloji polikliniğine 01.09.2000-30.11.2000 tarihleri arasında abortus imminens bulgularıyla başvuran 229 olgu değerlendirildi.Bu olgulardan 69 tanesi;yetersiz izlem veya birinci trimesterde olmaması nedeniyle çalışma dışı bırakıldı.Birinci trimesterde (12.gebelik haftası ve öncesinde) vaginal kanama yakınmasıyla başvuran olgulara, en az iki kez Kranz Büchler Sonoscop 3G 3.5 mHz.Abdominal brob ile ultrasonografik inceleme yapıldı. Bulgular:Abortus imminensli 160 hastanın yaş ortalaması 28' dir (17-43).Ortalama gravida 2, oratalama parite 1' dir.Olguların hepsinde hastaneye başvurma şikayeti 1 ila 9 gündür devam eden vaginal kanamadır.Olguların %45' inde (72/160) alt karın ağrısı ve abdominal kramplar mecvuttur.İncelenen olguların; %56.2' si (90/160) sağlam gebelik, % 22.5' i (36/160) inkomplet abortus, %11.8' i (19/160) missed abortus,% 8.75' i (14/160) anembriyonik gebelik, %0.75' i (1/160) ektopik gebeliktir.Ultrasonografik olarak gestasyonel yaşları ortalama 7.6' dır.(7 hafta 6 gün) Sonuç:Birinci trimester de ultrasonografik inceleme;kadın sağlığı morbidite ve mortalitesi açısından büyük önem taşımaktadır.
