Congenital hypothyroidism: Comparison of radioisotope and ultrasound imaging

Abstract
AIM: Thirty-three children suffering from congenital hypothyroidism were studied to determine the diagnostic impact of thyroid scintigraphy in comparison to ultrasound of the neck. MATERIAL AND METHODS: Thyroid scintigraphy ( with technetium-99m sodium pertchnetate (Tc-99m) or 123 iodine) and ultrasound of the neck were performed in all patients diagnosed as congenital hypothyroidism to evaluate anatomy and morphology of the thyroid gland. Ultrasound findings and radionuclide scanning were compared. RESULTS: Thyroid agenesis was diagnosed in 15 patients ectopic thyroid disease in 16 patients (11 sublingual, 3 lingual, 1 cervical, 1 submandibular thyroid). In these patients, no thyroid tissue was detected in the nech by ultrasound imaging. In two patients hypo plastic thyroid tissue were diagnosed by both ultrasound and scintigraphy. CONCLUSION: These findings suggest that scintigraphy is the best diagnostic technique for determining the anatomy of thyroid tissue. Ultrasound of the neck has only limited value in the assessment of ectopic thyroid tissue and distinguishing between ectopia and aplasia. But it may be used as a good complementary method to confirm the diagnosis and etiology of congenital hypothyroidism.

AMAÇ: Konjenital hipotiroidizmden şüphe edilen 33 hasta çocukta servikal ultrasonografi ve tiroid sintigrafisinin klinik kullanılabilirliğini araştırmak. GEREÇ VE YÖNTEM: Konjenital hipotiroidizm ön tanısı konan bütün hastalar tiroid anatomisi, morfolojisi ve fonksiyonları için ultrasonografik ve sintigrafik yöntem (teknisyum 99m sodyum perteknetat (Tc-99m) veya iyot 123) ile değerlendirildi. Ultrasonografik bulgular ve sintigrafik bulgular karşılaştırıldı. BULGULAR: Sintigrafi ile 15 hastada tiroid agenezi saptandı. Onaltı hastada ektopik yerleşimli tiroid bezi bulundu (11 sublingual, 3 lingual, 1 servikal, 1 submandibular). Bu hastaların ultrasonografilerinde tiroid bezi görülmedi. Ultrasonografi ile hipoplazi saptanan 2 hastanın tanısı sintigrafi ile doğrulandı. SONUÇ: Radyoizotop tiroid sintigrafisi tiroid bezi anatomisini en iyi belirleyen tanısal yöntemdir. Ultrasonografi ektopiyi tanımlamada ve ektopiyi aplaziden ayırmada tek başına yeterli değildir. Ancak konjenital hipotiroidizm etyolojisinin belirlenmesinde yardımcı bir teknik olarak kullanılabilir.
