The effect of cigarette smoking on bone mineral density in postmenopausal women

Abstract
AIM: In women, the major cause of bone loss and osteoporosis is estrogen withdrawal, most commonly associated with the menopause. There are many conditions other than estrogen deficiency that cause bone loss, such as metabolic abnormalities, malignancies and exposure to certain drugs. Smoking is also considered as a risk factor that may lead to accelerated bone loss with consequent osteoporosis. In this study we investigated whether bone mineral density is effected with cigarette smoking in postmenopausal patients. MATERIALS AND METHODS: The study is performed with the retrospective analysis of 1068 (966 non-smoker, and 102 smoker) patients currently on follow-up in our osteoporosis outpatient clinic. Two groups were similar with regard to age at menopause and duration of menopause. Diagnosis of osteoporosis and osteopenia was based on lumbar bone mineral density (BMD) levels. RESULTS: The mean age and mean body mass index of the smoker group was lower than that of the non-smokers (56.0 ± 9.2 vs. 59.0 ± 8.9, p<0.0001; and 27.3 ± 4.3 vs. 29.2 ± 4.6, p<0.001, respectively). Bone mineral density was significantly lower in smokers (0.797 ± 0.114 vs. 0.832 ± 0.153 g/cm2, respectively; p=0.02). Bone mineral density was significantly negatively correlated to duration of smoking and number of cigarettes comsumed per day (r = -0.316, p = 0.001; and r = -0.371, p<0.001; respectively). CONCLUSION: Our data show that cigarette smoking decreases bone mineral density depending on both the duration of smoking and number of cigarettes comsumed per day in postmenopausal women.

AMAÇ: Kadınlarda kemik kaybının başlıca nedeni östrojen hormon eksikliğidir ve çoğunlukla menopozla birliktedir. Ostrojen eksikliğinden başka, metabolik hastalıklar , tümörler, ilaçlar ve diğer durumlar da kemik kaybına neden olurlar. Sigara içimi de kemik kayıp hızını artırıp osteoporoz oluşumuna neden olan bir risk faktörü olarak kabul edilmektedir. Bu çalışmada postmenopozal kadınlarda kemik mineral yoğunluk değerinin sigara içimi ile etkilenip etkilenmediğini araştırdık. GEREÇ VE YÖNTEM: Çalışma osteoporoz polikliniğine başvuran 1068 (966 sigara içmeyen ve 102 sigara kullanan) postmenopozal kadın hastanın retrospektif olarak değerlendirilmesi ile gerçekleştirildi. İki grubun menopoz yaşı ve menopoz süresi benzerdi. Osteoporoz ve osteopeni tanısı lomber kemik mineral yoğunluğu (KMY) değeri ile saptandı. BULGULAR: Sigara içen grubun yaş ve beden kitle endeksi ortalamaları sigara içmeyenlerden daha düşük idi (sırasıyla 56.0 ± 9.2'e karşı 59.0 ± 8.9, p<0.0001 ve 27.3 ± 4.3'e karşı 29.2 ± 4.6, p<0.001). Sigara içen grupta kemik mineral yoğunluğu (KMY) değeri anlamlı olarak düşük saptandı (0.797 ± 0.114'e karşı 0.832 ± 0.153 g/cm2, p=0.02). Sigara alışkanlığı olan hastalarda sigara kullanma süresi ve günde içilen sigara sayısı ile KMY arasında anlamlı negatif korelasyon saptandı (sırasıyla r = -0.316, p = 0.001; ve r = -0.371, p<0.001). SONUÇ: Verilerimiz sigara kullanımının postmenopozal kadınlarda kemik mineral yoğunluğunu hem kullanım süresi hem de içilen sigara miktarına bağlı olarak azalttığını göstermektedir
