Stevens Johnson Syndrome resulting from radiation and fhenytoin: A case report

Abstract

Stevens Johnson Syndrome (SJS) is severe exfoliative dermatitis accompanied by fever, inflammation of the gasrointestinal mucosa and severe purulent conjunctivitis. Drugs that can also precipitate SJS include sulfonamides, penicilins, phenylbutazone and phenytoin .Thus this is a clinical phenomenon that occurs more frequent in patients with brain tumor who under goradiation therapy while taking phenytoin. 49 - year- old male patient was admitted to hospital presenting symptom and findings with fever, malasia, rapidly developing blistering exanthema of macules, target- like lesions well demarcated plaques especially on the head, neck skin and the other side of body accompanied by mucosal involvement. We report a case of severe SJS, which occurred in e patient, with metastatic squamous cell carcinoma receiving phenytoin and whole-brain radiation therapy (WBRT). In the absence of seizures, anticonvulsants should not be given routinely to patients with brain tumors.. When anticonvulsants are necessary in patients scheduled for WBRT, phenytoin may not be the drug of choice but valproic acid may be more confident, The pathogenesis of the this interaction will be discuss briefly.

Özet

Stevens Johnson Sendromu (SJS) şiddetli eksfoliatif dermatit,ateş,gastrointestinal sistem inflamasyonu ve pürülan konjuktivit birlikteliğinden oluşan bir sendromdur. Eritema multiforme major olarak da isimlendirilir.Eriteme multiformeyi presipite eden faktörler arasında sulfonamidler, penisilinler,fenilbutazon ve fenitoin sayılabilir.Beyin tümörü nedeniyle radyoterapi ve fenitoin tedavisi gören hastalarda görülme sıklığı artmaktadır. Skuamöz hücreli akciğer karsinomunun beyin metastazı nedeniyle kranial radyoterapi ve fenitoin alan bir hastada gelişen şiddetli SJS olgusunu rapor etmek istiyoruz. 49 yaşında erkek hasta ateş, halsizlik,özellikle baş ve boyunda gelişen egzantemli maküller,iyi sınırlı plak tarzında target lezyonlar ve mukozal tutulum ile kabul edildi.Beyin tümörü olan hastalara konvulsiyonlar olmadıkça profilaksi için rutin olarak antikonvulsan verilmemelidir. Kranial radyoterapi gören hastalara antikonvulsan vermek gerekirse ilk seçenek fenitoin olmamalı,bunun yerine valproik asit kullanılmalıdır. Bu etkileşim patogenezi kısaca tartışılacaktır.
