Violence against gyncologists and obstetricians

Abstract
Objective: Violence to physicians is frequently encountered in the recent time and this topic is being discussed seriously in press. Our aim was to investigate verbal and physical assaults related to our profession. Material and Methods: We made a questionnaire with 110 gynecologist and obstetricians (69 assistant doctors and 20 specialists) in three different hospitals of Ankara (Community Hospital, Social Assurance Departement-SAD-Hospital and University Hospital). Ages of doctors were ranging between 24 and 52. Most of the physicians were in the second year of their training. 17 questions and related subtopics were asked and the answers were recorded. Results: 80% (n=89) of physicians declared that they were exposed to assault from patients or their relatives at least once during the last year. The most encountered assault was the verbal one (89%, n=80). Others, in decreasing order, were being threatened (43%, n=39), attempting to physical assault (21%, n=19), physical assault (7.8%, n=7) and being injured by an incisive instrument or by a firearm (1.1%,n=1), respectively. Of physicians being assaulted, 60% were female and 40% were male. 85% of participant women were assaulted.whereas this ratio was 74% in male participants. Violence was mostly sxposed in hospitals (94%,84 of 89 physicans). Violence ratios were 86%, 83% and 57% in SAD Hospital, Community Hospital and University Hospital, respectively. Interestingly, few events had been reported to hospital direction and/or police département (31%, n=28). Most physicians expressed that they went on treating the patient (85%), whereas 7% halted dealing with the patient, 7% advised another gynecologist/obsetrician and 7% of physicians declared that family of the patient requested another doctor. 87% of physicians claimed the family of the patient, whereas 13% declared that they were erroneous together with the family According to the physicians'opinion; moral insufficiency (83.6%), the influence of press (83.6%), lack of confidence to the doctor (55.4%) and low sociocultured status of the family (30%) determined the behaviour of patients and their families. Factors determining the physician related faults were found to be lack of sufficient number of staff (63.6%), staff's low educational level (63.6%), tiredness of doctors (59%), lack of enough time to care for the patient (55%), phisical incapability of the hospital (46%), physicians' economical problems (37%), intolerance of physicians (15%) and educational incapability of physicians (12%). Physicians reported that they had got almost no education for such an event (95%, n=105). However, 21% of physicians explained that they also do not require education about this subject. 73% of physicians expressed that they were influenced negatively by such an event. Conclusion: We conclude that gynecologist and obsetricians are under serious threatening of violence by patients and their families. Also it appears that lack of enough education about this subject, both for patients and physicians, is a determining factor for consequences. Taking up the subject with a broad perspective can be helpful for the solution.

Amaç: Hekime karşı şiddet son zamanlarda sıkça karşımıza çıkmakta ve basın yayında da oldukça ciddi yer bulmaktadır. Mesleğimiz dolayısıyla karşımıza çıkan sözlü-fiziki saldırıyı incelemek için bu çalışma yapıldı. Gereç ve Yöntem: Ankara'nın üç farklı hastanesinde (Devlet ve SSK Eğitim Hast, ve Üniversite Kadın Hastalıkları Doğum Klinikleri) 110 kadın doğum hekimi ile önceden hazırlanan formlar doldurularak anket yapıldı. Bu ankette 17 soru ve alt maddelerine cevap arandı. Sonuçlar: Ankete katılanların %80'i (n=89) hasta veya yakınları tarafından, en az bir kere ve son bir yılda daha sık olmak üzere şiddete maruz kaldıklarını (SMK) belirttiler. En sık karşılaşılan durum %89 (n=80) sözle rahatsız edilme idi. Diğerleri sırasıyla tehdit %43 (n=39), fiziki saldırıya yeltenme %21 (n=19), fiziki saldırı %7.8 (n=7), kesici- delici - silahlı alet ile yaralanma %1.1 (n=1) olarak tespit edildi. Şiddete maruz kalanlarda kadın oranı %60, erkek oranı % 40, ankete katılan kadınlarda ŞMK'ların oranı %85 erkeklerden ŞMK'ların oranı %74 idi. Şiddetle çoğu defa hastanede karşılaşıldı, %94 (84/89). Şiddet oranı SSK hastanelerinde %86, devlet hastanelerinde %83, üniversite hastanelerinde %57 idi. İlginç olanı, bu durumları hastane idaresi ve /veya polise bildirme oranının düşüklüğü idi %31 (n=28). Hekimlerin çoğu (%85) olay sonrası aynı hastaya bakmaya devam ettiklerini, %7'si hastaya bakmadıklarını, %7'si başka hekime gönderdiklerini, %7'si ailenin kendi isteği ile başka hekime gittiğini bildirdi. Hekimlerin %87'si hatanın ailede, %13 ise aile ile beraber kendilerinde de olduğunu ifade etti. Şiddette, ailenin hatasının en çok ahlaki yetersizlik sonucunda %83.6, basın yayının etkisi ile %83.6, ailenin hekime güvensizliği sonucu %55.4, ve en az olarak da ailenin akademik olarak eğitiminin az oluşu ile %30 olduğu belirtildi. Hekim ve çevresi ile ilgili hatalarda ise sırası ile etkili faktörler, yardımcı ekibin sayıca yetersizliği %63.6, bu ekibin eğitimce eksikliği %63.6, hekimin yorgunluğu %59, hastaya yeterli süre ayrılmaması %55, hastanenin fiziki yetersizliği %46, hekimin maddi olarak sıkıntıda olması %37, hekimin hoşgörülü olmaması %15, hekimin eğitimindeki eksiklik %12.7 olarak bulundu. Hekimler bu konuda hemen hemen hiç eğitim almadıklarını belirttiler %95 (105/110). Ancak bu konuda eğitim almayı isteme konusunda hevesli olmayanlar da vardı, (isteyenler %79, istemeyenler %21). Hekimlerin %73'ü bu olaylardan olumsuz etkilendiklerini ifade etti. SMK hekimlerin 69'u araştırma görevlisi (%77.5) 20'si uzman hekim idi (%22.5). SMK hekimlerin çoğunluğu 24-30 yaş grubu içinde yer aldı. Bu hekimlerin çoğunluğu ikinci asistanlık yılları içindeydi. Tartışma: Kadın hastalıkları ve doğum hekimine yönelik şiddetin boyutunun önemli olduğunu, hasta ve ailesi ile hekimin bu konuda eğitim eksikliğinin, sonucu belirleyici faktör olarak ortaya çıktığı, konunun hekim ve hasta ailesi açısından geniş bir çerçevede ele alınması gerektiğini, bu konuda tedbir alınması ve tıp eğitiminde bu konuya yer verilmesinin gerekli ve yararlı olacağını düşünüyoruz.
