The importance of Douglas cytology in differential diagnosis of pelvic masses

Abstract
AIM: Althoug there are many currently in-use-methods in differantial diagnosis of pelvic masses, there is still a need of surgical and pathologic findings in differantial diagnosis of benign and malignant. We tried to establish the diagnosis of benign-malignant types of pelvic masses preoperatively, with this study. INSTITUTION: SSK Agean Maternity Hospital, İZMİR. MATERIAL AND METHOD: Culdosentesis were applied with 20 G spinal needle to 92 patients who were hospitalized in SSK Agean Maternity Hospital Gynecology and Oncology departmants in between October 1997- June 1999 and who had taken the operation indication. The fluid that were taken, were sent to the pathology laboratory in syringes. CONCLUSION: 47 of the 92 patients were from gynecology department and the remaining 45 were from oncology department. 30 (32.6%) of these 92 patients were in postmenapausal period and the remaining 62 (62.4%) were in premenapausal period. With the culdosentesis method,a maximum of 25 cc and a minimum of 1 cc fluid could be taken. The cytology of these were as follows: 3.2% malignant, 2.1% no cell observed, 1.1% not proper and all the remaining were benign. Specifity and sensitivity calculated from all of the patients were 100% and 6.5%, respectively. Specifity and sensitivity of 8 patients with abdominal ascites were 100% and 40% whereas in 84 patients without abdominal ascites, the valves were 100% and 2.4% respectively. RESULTS: The sensitivity of culdosentesis cytology in differantial diagnosis of benign or malignant pelvic masses is very low. And additionally, gives no clues about the diagnosis of the disease. But it is a specific method. Althoug obtaining a malignant result possibility with culdosentesis cytology is limited, if a malignant result is obtained, it is reliable.

AMAÇ: Pelvik kitle ayırıcı tanısında kullanılan birçok yöntemler olmasına rağmen,benign ve malign ayırımında hala cerrahi ve patolojik bulgulara ihtiyaç vardır. Biz de bu çalışmamızda, pelvik kitlelerde girişim öncesi Douglas sitolojisinin benign-malign ayırımındaki yerini saptamaya çalıştık. GEREÇ VE YÖNTEM: Ekim 1997 - Haziran 1999 tarihleri arasında SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi Jinekoloji ve Onkoloji servislerine pelvik kitle öntanısı ile yatan ve operasyon endikasyonu verilen 92 hastaya 20 G spinal iğne ile kuldosentez uygulandı. Alınan mai enjektör içinde patoloji laboratuarına gönderildi. BULGULAR: 47' si jinekoloji ve 45' i onkoloji servisinde izlenen toplam 92 hastanın 30' u (%32.6) postmenopozal ve 62' si premenopozal (%62.4) dönemde idi. Kuldosentez işlemi ile max. 25 cc ve min. 1 cc mai alınabildi. Bunlardan % 3.2' si malign, %2.1 'i hücre izlenmedi, %1.1' i niteliksiz ve geri kalanların hepsi benign özellikte idi. Tüm hastalar göz önüne alındığında spesifisite %100 ve sensitivite % 6.5 olarak hesaplandı. Batında asit olan 8 vakada spesifisite %100, sensitivite %40 iken asit olmayan 84 hastada spesifisite %100, sensitivite %2.4 olarak bulundu. SONUÇ: Kuldosentez sitolojisinin kitlenin benign-malign ayırımında sensitivitesi oldukça düşüktür. Ayrıca hastalığın tanısına yönelik ip uçları vermemektedir. Fakat spesifik bir yöntemdir. Kuldosentez sitolojisi ile malign sonuç elde etme olanağı sınırlı olmakla birlikte, malign sonuç geldiğinde güvenilirdir.
