Prevention of postoperative nausea and vomiting using ondansetron: A dose ranging study

Abstract

AIM: Postoperative nausea and vomiting are common after recovery from general anesthesia especially in patients undergoing gynecologic surgery. 60 patients aging between 18-65 undergoing vaginal hysterectomy were studied. MATERIAL AND METHODS: The antiemetic effect of ondansetron, a selective serotonin type 3 (5-HT3) receptor antagonist, was determined by giving a single intravenous dose of ondansetron (4 mg , n=20 and 8 mg, n=20) or placebo (n=20) before the induction of anesthesia in a randomized, double-blind manner. A standardized anesthetic technique consisting of fentanyl-thiopental-atracurium for induction and nitrous oxide-isoflurane for the maintenance of anesthesia was used. Nausea and vomiting was observed 24 hours after the operation in the postoperative period. A rescue antiemetic was provided in case of continued vomiting. Antiemetic efficacy was scored as complete, major, minor and inadequate. RESULTS: At the end of 24 hour postoperatively, ondansetron (4 and 8 mg) significantly decreased the postoperative nausea and vomiting incidence versus placebo. 25% and 20% of patients experienced nausea and vomiting in ondansetron groups (4 and 8 mg respectively) versus 55% of patients in placebo group (p<0.01). 11 of the placebo-treated patients required a "rescue" antiemetic compared with 5 and 4 patients in the ondansetron groups (4 and 8 mg respectively) (p<0.05). CONCLUSION: Ondansetron at a dose of 4 or 8 mg adminestered intravenously before the induction of anesthesia appears to be an effective antiemetic for the treatment of postoperative nausea and/or vomiting after vaginal hysterectomy.

Özet

AMAÇ: Genel anesteziden sonra postoperatif bulantı ve kusma, özellikle jinekolojik operasyonlardan sonra sıklıkla gözlenir. Bu çalışmada majör jinekolojik operasyon geçirecek hastalarda POBK profilaksisi için verilen ve selektif serotonin tip 3 (5-HT3) reseptör antagonisti olan tek doz ondansetronun etkinliği farklı dozlar kullanılarak karşılaştırıdı. GEREÇ VE YÖNTEM: Yaşları 18-65 arasında değişen ve vajinal histerektomi geçirecek 60 hasta çalışmaya alındı. Hastalar rastgele 3 gruba ayrıldı. Anestezi indüksiyonundan 2 dk. önce intravenöz yoldan I. gruba (n= 20) 4 mg ondansetron, II. gruba (n= 20) 8 mg ondansetron ve III. gruba (n= 20, plasebo) 2 ml salin uygulandı. Anestezi indüksiyonunda Na tiopental, fentanyl (1mgr/kg) ve kas gevşetici olarak da atrakuryum uygulandı. Anestezi idamesinde ise %50 O2 içinde İsofluran/N2O kombinasyonu kullanıldı. BULGULAR: Hastalar postoperatif dönemde 24 saat süreyle bulantı-kusma açısından takibe alındı. Çalışma ajanına rağmen devam eden bulantı-kusma söz konusu ise ek doz antiemetik-metoklopramid uygulandı. Uygulanan skorlama sistemine göre alınan yanıt tam, majör, minör ve başarısız olarak değerlendirildi. Postoperatif 24.saatin sonunda bulantı-kusma insidansı 4 ve 8 mg ondansetron verilen gruplarda sırasıyla %25 ve %20 iken plasebo grubunda %55 bulundu (p<0.01). Ek doz antiemetik ihtiyacı plasebo grubunda (11 hasta) diğer iki gruptan (I.grupta 5 ve II. grupta 4 hasta) daha fazla olduğu saptandı (p<0.05). SONUÇ: Bulgularımız majör jinekolojik operasyon geçiren hastalarda 4 veya 8 mg ondansetronun POBK'yı önemli oranda önlediğini göstermektedir.
