Cyclophosphamide treatment in steroid resistant nephrotic syndrome

Abstract
Some of the nephrotic syndromes of childhood are resistant to steroids. For this group of children, the alternative drugs(Alkalizing agents, cyclosporin-A, etc.) can be used. The prognosis for steroid resistant patients is poor. In this study, the effectiveness of cyclophosphamide treatment for steroid resistant nephrotic sendromes and the short term adverse effects of the drug, are investigated. The study is worked out through 1/1/1985 and 1/1/1996, including 19 patients with steroid resistant nephrotic syndrome, mean age 7.34 ± 4 years(age distribution 1-14 years) 9 female, 10 male. The patients are given 2 mg/kg/day(max: 60 mg/day) prednisolone for 6-8 weeks None of them improved so they are considered as prednisolone resistant patients. They are given 2.5 mg/kg/day cyclophosphamide for 8 weeks, and investigated for response to the drug and for the short term adverse effects (supression of the bone marrow, hemorrhagic cystitis, irritation of gastrointestinal tract, infections, alopecia, etc.). Response to cyclophosphamide was observed in 12 of 19 patients(%63.15), seven patients(%36.85) were unresponsive and 1 of the 19 patients(%5.26) had infections during therapy. No other acute adverse effects of the drug was seen. Four patients who were found to have minimal change disease(MLH) by renal biopsy improved by cyclophosphamide therapy. The response to cyclophosphamide was poor in patients with glomerulopathies other than MLH. No other differences were found between the groups that responded and not responded to cyclophosphamide. In conclusion, cyclophosphamide treatment for steroid resistant nephrotic syndromes, especially for steroid resistant MLH, is found to be an effective therapy. Acute complications did not occur during the therapy. Response to cyclophosphamide was poor in glomerulopathies other than MLH.

Çocukluk çağı nefrotik sendromunun bir kısmı steroide dirençlidir. Bu grup hastaların tedavisinde diğer alternatif ilaçlar(Alkilleyici ajanlar, Siklosporin A vs.) kullanılabilir. Prognoz ise steroide duyarlı olgulara göre daha kötü seyretmektedir. Bu çalışmada, steroide dirençli nefrotik sendromlu olgularda siklofosfamid tedavisinin etkinliği ve ilacın kısa süreli yan etkisi araştırılmak istenmiştir. Çalışma, 1/1/ 1985-1/1/ 1996 yılları arasında hastanemizde yatan, yaş ortalaması 7.34±4 yıl(yaş dağılımı 1-14 yıl) olan 9 kız, 10 erkek, toplam 19 steroide dirençli olgu üzerinde yapılmıştır. Olgular 2mg/kg/gün(max:60mg/gün) 6-8 hafta prednizolon kullanmasına rağmen remisyona girmemiş ve steroide dirençli kabul edilmiştir. Bu olgulara 2.5 mg/kg/gün siklofosfamid 8 hafta süreyle kullanılmış ve olgular siklofosfamide yanıt ve kısa süreli yan etkileri yönünden(Kemik iliği supresyonu, hemorajik sistit, gastrointestinal sistem irritasyonu, enfeksiyonlar, alopesi, vs.) izlenmiştir. Ondokuz hastadan 12 sinde (%63.15) siklofosfamide yanıt saptanmış, 7(%36.85) hastada siklofosfamide yanıt alınamamıştır. Hastaların sadece 1'inde(%5.26) tedavi sırasında enfeksiyon saptanmıştır. İlacın diğer akut yan etkileri gözlenmemiştir. Böbrek biyopsisinde minimal lezyon hastalığı (MLH) olan 4 olguda siklofosfamid tedavisine yanıt elde edilmiştir. MLH dışında glomerülopati saptanmış ise siklofosfamid tedavisine yanıtın az olduğu gözlenmiştir. Siklofosfamide yanıt alınan ve alınmayan gruplar arasında başka herhangi bir fark bulunmamıştır. Sonuç olarak, steroide dirençli nefrotik sendromlularda ve özellikle steroide dirençli MLH'da alkilleyici ajanlardan siklofosfamid tedavisinin etkin bir tedavi olduğu, tedavi sırasında akut komplikasyonların sık görülmediği, MLH dışı glomerülopatilerde siklofosfamid kullanımının yetersiz kaldığı gözlenmiştir.
