Comparison of two different hepatitis B vaccinal schedules in healthy infants

Abstract
Objective: Early vaccination after birth is of great importance for prevention of perinatal hepatitis B infection in developing countries in which rate of carriers is high. In this study, anti-HBs responses to hepatitis B vaccination, given to healthy newborns on birth, first, and sixth months or second, fourth, and sixth months are tested. Material and Method: A total of 73 babies were included in two groups in the study. There were not any significant differences in age, gender, and weight between the two groups. Group I (n=34) at birth, first, and sixth months and group II (n=39) at second, fourth, and sixth months received 10 mcg hepatitis B vaccine (Engerix B, pediatric, Smith Kline Beecham). Result: Sera were obtained three months after the last dose of vaccination and tested for antibodies against hepatitis B surface antigen by ELISA test kit. Seroconversion and seroprotection rates were, 100 %, and 85.3 % in group I, and 100 %, and 97.5 % in group II respectively. Anti-HBs levels above the 100 mIU/ml level were significantly higher than the 1-9 mIU/ml and 10-100 mIU/ml levels in group II compared to the levels in group I (p= 0.020, p=0.0008). Seroconversion and seroprotection rates were not significantly different between the two groups. Conclusion: Early vaccination scheme is an effective measure against HBV infections in Turkey where hepatitis B serology tests are not routinely performed; however delayed vaccination scheme produces higher antibody levels than early vaccination.

Amaç: Taşıyıcılık oranının yüksek olduğu gelişmekte olan ülkelerde tüm yenidoğanların doğumdan hemen sonra aşılanması perinatal hepatit B enfeksiyonun önlenmesinde büyük önem taşımaktadır. Çalışmamızda sağlıklı yenidoğanların 0, 1, 6. ve 2, 4, 6. aylarda yapılan hepatit B aşısına karşı gelişen anti-HBs yanıtlarını araştırdık. Gereç ve Yöntem: Çalışmaya alınan 73 bebek iki gruba ayrıldı. Gruplar arasında yaş, cins ve ağırlık açısından istatiksel olarak anlamlı fark yoktu. I. gruba (n= 34) 0, 1, 6. aylarda ve II. gruba (n= 39) 2, 4, 6. aylarda 10 mcg dozunda hepatit B aşısı (Engeriks B, pediatrik, Smith Kline Beecham) rekombinant hepatit B virus aşıları uygulandı. Bulgular: Serum örnekleri son aşılamadan 3 ay sonra alınarak hepatit B yüzey antijenine karşı gelişen antikor düzeyleri ELİSA test kiti ile saptandı. Serokonversiyon ve seroproteksiyon oranları sırası ile Grup I'de % 100, % 85.3 ve grup II'de % 100 ve % 97.5 olarak bulundu. İki grup kıyaslandığında, grup II'deki 100 mIU/ml'nin üzerindeki anti-HBs düzeylerinin 1-9 mIU/ml ve 10-100 mIU/ml'lik anti-HBs düzeylerine göre anlamlı şekilde daha yüksek olduğu gözledi (p= 0.020, p= 0.0008). Serokonversiyon ve seroproteksiyon oranları bakımından ise her iki grup arasında istatistiksel olarak anlamlı bir fark saptanmadı. Sonuç: Erken aşılama şemasının annenin hepatit B serolojisine rutin olarak bakılamadığı ülkemiz koşullarında HBV infeksiyonlarına karşı etkili bir koruma sağladığı, ancak geç aşılamanın daha yüksek antikor düzeyleri oluşturduğu sonucuna varıldı.
