The effect of pentoxifyilline on peripheral neuropathy in patients with diabetic foot ulcers

Abstract

AIM: Diabetic foot is a prevalent and potentially grave complication of diabetes mellitus. Peripheral neuropathy is one of the most important etiologic factors in developing foot ulcers. Microvascular disease of the vaso nervosum has recognised as a contributing or primary factor in the development of diabetic neuropathy. Pentoxifylline has been found to improve blood supply through partially occluded vessels. The purpose of this study was to determine the effect of pentoxifylline treatment on diabetic neuropathy. MATERIALS AND METHODS: Twenty-nine patients with Type II diabetes mellitus were included in this study. All patients had diabetic foot ulcers. Group I was control consisted of 15 patients. Fourteen patients were included in Group II eksperimental group and they were given pentoxifylline for 6 months, 1200 mg/day i.v. in the first two weeks and than 1200 mg/day po. Electromyographic examinations were performed for all the patients at the beginning of the study, and after six months. RESULTS: Electrophysiological examinations showed that after six months in Group I sensorial nerve conduction velocity and sensorial nerve action potantial amplitude was measured to be decreased (p<0.05), however in Group II compound muscle action potantial amplitude calculations were found to be increased after pentoxifylline treatment (p<0.05). CONCLUSION: Six months of pentoxifylline therapy slowed down the course of diabetic neuropathy and increased asonal reinnervation.

Özet

AMAÇ: Diabetik ayak diabetes mellitusun yaygın ve ciddi bir komplikasyonudur. Ayak ülserlerinin gelişmesinde periferik nöropatinin etkisi büyüktür. Vazo nervorumlarda iskemi ile sonuçlanan mikrovasküler hasar diabetik nöropati gelişiminden sorumlu olabilir. Parsiyel olarak tıkalı damarlarda kan akımını artırdığı gösterilmiş olan pentoksifillinin bu aşamada faydalı olabileceği ileri sürülmüştür. Bu çalışmanın amacı diabetik ayak komplikasyonu gelişmiş olan Tip II diabetes mellituslu hastalarda pentoksifillinin periferik nöropati üzerine olan etkisini incelemektir. GEREÇ VE YÖNTEM: Diabetik ayak ülserleri olan Tip II diabetes mellituslu 29 hasta çalışmaya dahil edildi. Hastalar rastlantısal olarak 2 gruba ayrıldılar. Grup I (kontrol grubu), 15 hastadan oluştu. Grup II'de (deney grubu) ise 14 hasta yer aldı. Deney grubundaki hastalara 6 ay süreyle pentoksifillin tedavisi uygulandı. Pentoksifillin ilk 2 hafta 1200 mg/gün i.v. olarak, daha sonra aynı dozda oral tedaviye geçilerek kullanıldı. Bütün olgulara ilk başvuruda ve 6 ay sonunda elektromyografik ölçümler yapıldı. BULGULAR: Elektrofizyolojik çalışmalar değerlendirildiğinde altı ay sonunda Grup I'deki hastalarda duyusal sinir iletim hızında ve duyusal sinir aksiyon potansiyel amplitüdünde istatistiksel olarak anlamlı azalma saptanırken (p<0.05), Grup II'deki hastalarda pentoksifillin tedavisinden sonra birleşik kas aksiyon potansiyel amplitüdünde istatistiksel olarak anlamlı artış saptandı (p<0.05). SONUÇ: Altı ay süreyle uygulanan pentoksifillin tedavisi diabetik nöropatinin seyrini yavaşlatmış ve aksonal reinnervasyonda artışa neden olmuştur.
